自我效能在食管癌术后病人希望与乐观应对间的中介效应

摘　　要：[目的]了解食管癌术后病人自我效能、希望和乐观应对状况,探讨食管癌术后病人自我效能在希望及乐观应对中的中介效应。[方法]2017年9月—2018年3月便利抽样法选取山东省某三级甲等医院221例食管癌手术后病人为研究对象,采用一般资料调查表、Herth希望量表、癌症自我效能感量表、Jalowiec应对量表进行问卷调查。研究采用描述性分析、Pearson相关分析和中介效应分析。[结果]食管癌术后病人希望总分为(34.91±4.09)分,自我效能得分为(95.32±14.31)分,乐观应对得分为(2.50±0.31)分。希望、自我效能与乐观应对均呈正相关(P<0.001)。[结论]食管癌术后病人希望及自我效能处于中等水平,且均为乐观应对的保护因素。自我效能在希望和乐观应对的关系中起到完全中介效应。增强自我效能和希望是促进食管癌术后病人乐观应对能力的有效途径。护理人员应该在病人住院期间引导培养术后新生活的信心。Objective:To explore the current situation of the self-efficacy,hope and optimistic coping responses in postoperative esophageal cancer patients,and to explore the mediating effect of self-efficacy in hope and optimistic coping responses in postoperative esophageal cancer patients.Methods:A total of 221 postoperative patients with esophageal cancer were selected as the research objects by the convenience sampling method in a third-class A level hospital in Shandong Province from September 2017 to March 2018.The general information questionnaire,Herth Hope Index(HHI),Strategies Used by People to Promote Health(SUPPH)and the Jalowiec Coping Scale(JCS)were used to conduct questionnaire survey.The descriptive analysis,Pearson correlation analysis and mediating effect analysis were used for data processing.Results:The total scores of HHI,SUPPH and JCS were(34.91±4.09),(95.32±14.31)and(2.50±0.31)in postoperative esophageal cancer patients,respectively.Hope,self-efficacy and optimistic coping responses were all positively correlated(P<0.001).Conclusions:The hope and self-efficacy in postoperative esophageal cancer patients are at a moderate level,and they are both protective factors for optimistic coping responses.Self-efficacy plays the completely intermediary role in the relationship between hope and optimistic coping responses.Enhancing self-efficacy and hope can promote optimistic coping responses in postoperative esophageal cancer patients.Nurses should guide and cultivate self-efficacy in hospitalized patients.
