甲功五项及外周血TPOAb、TGAb水平在早孕期稽留流产中的预测价值评估

摘　　要：目的评估甲功五项及外周血甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)水平在早孕期稽留流产(MA)中的预测价值。方法选取检出甲功五项或外周血TPOAb、TGAb水平异常的早孕期孕妇80例为研究组,同时选取甲功五项和外周血TPOAb、TGAb水平均正常的早孕期孕妇80例为对照组。观察比较两组孕妇的MA发生率以及发生MA妇女与未发生MA孕妇的甲功五项和外周血TPOAb、TGAb水平异常情况。结果研究组的MA发生率显著高于对照组(P<0.01);发生MA妇女的甲亢发生率明显高于未发生MA孕妇(P<0.05),且甲减、亚甲减、低FT4以及桥本病发生率均显著高于未发生MA孕妇(P<0.01),而亚甲亢发生率与未发生MA孕妇无统计学差异(P>0.05)。结论甲亢、甲减、亚甲减以及低FT4和桥本病均可增高早孕期MA的发生率,甲功五项和外周血TPOAb、TGAb水平对早孕期MA具有预测价值。Objective To evaluate predictive value of five thyroid analytes,levels of thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TGAb)in peripheral blood for missed abortion(MA)during early pregnancy.Methods 80 early pregnant women with abnormal results of five thyroid analytes,TPOAb and TGAb in peripheral blood in the hospital were enrolled as study group.Eighty early pregnant women with normal results were enrolled as control group.The incidence of MA between the two groups,and abnormalities of five thyroid analytes,TPOAb and TGAb in peripheral blood between women with MA and without MA were observed and compared.Results The incidence of MA in study group was significantly higher than that in control group(P<0.01).The incidence of hyperthyroidism in women with MA was significantly higher than that without MA(P<0.05),incidence of hypothyroidism,subclinical hypothyroidism,low FT4 and Hash imoto’s disease was significantly higher than that without MA(P<0.01),while incidence of subclinical hyperthyroidism was not different from that without MA(P>0.05).Conclusion Hyperthyroidism,hypothyroidism,subclinical hypothyroidism,low FT4 and Hashimoto’s disease can increase incidence of MA during early pregnancy.The five thyroid analytes,and TPOAb and TGAb in peripheral blood are of predictive value for MA during early pregnancy.
