中医治疗糖尿病足筋疽研究进展

摘　　要：糖尿病足筋疽的治疗可分为内治法、外治法、祛腐清筋术以及传统针灸疗法,其中内治法包括口服糖足方、清消颗粒、陈兰花冲剂等,外治法分为紫朱软膏外敷、渴疽洗方外洗及负压封闭引流等。在中西医结合控制血糖的基础上,通过内治法与外治法结合控制感染,可改善局部血液循环和创面微环境以促进创面愈合。但是,糖尿病足筋疽发病机制还有待进一步研究,需明确微生物因素对创面的作用机制,也需从现代药理学和生物化学角度阐述治疗糖尿病足筋疽方剂的作用机制,探索中医特色疗法与现代医学的有机结合,以降低复发率、截肢(趾)率和死亡率,提高患者的生活质量。The treatment of diabetic foot tendon gangrene can be divided into internal treatment,external treatment,Master Xi′s method of removing rotting and clearing tendons,and traditional warming acupuncture and moxibustion therapy.The internal treatment includes Diabetic Foot Formula,Clearing and Removing Granule,Chen Lan Hua Granule(陈兰花冲剂,CLHG),etc.The external treatment includes Zi Zhu Ointment(紫朱软膏,ZZO),Diabetic Foot Washmittel,vacuum sealing drainage,etc.On the basis of controlling blood sugar by combining traditional Chinese and Western medicine,the infection can be controlled by combining internal and external treatment,which can improve local blood circulation and wound microenvironment to promote wound healing.However,the pathogenesis of diabetic foot gangrene needs to be further studied.It is necessary to clarify the mechanism of microbial factors on the wound surface.It is also necessary to elaborate the action mechanism of prescription for treating diabetic foot tendon gangrene from the perspective of modern pharmacology and biochemistry,and explore the organic combination of TCM and modern medicine,so as to reduce the recurrence rate,amputation rate and mortality rate,and improve the life quality of patients.
