摘　　要：目的探讨股骨近端抗旋髓内钉(PFNA)内固定治疗高龄股骨粗隆间骨折的临床效果。方法回顾性选取2018年7月~2019年12月我院收治的60例高龄股骨粗隆间骨折患者资料,按照研究方法的不同将其分为对照组与研究组,每组各30例。对照组采用股骨近端锁定钢板(LCP)进行治疗,研究组采用PFNA内固定术进行治疗,观察两组手术时间、住院时间、骨折临床愈合时间、术中出血量、Harris髋关节功能、视觉模拟量表(VAS)评分、并发症发生情况。结果研究组患者手术时间、住院时间、骨折临床愈合时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后2周、1个月时,研究组患者Harris髋关节功能评分高于对照组,差异有统计学意义(P<0.05);研究组术后VAS评分低于对照组,差异有统计学意义(P<0.05);研究组无并发症发生,对照组并发症总发生率为6.67%,两组比较差异无统计学意义(P>0.05)。结论PFNA内固定治疗高龄股骨粗隆间骨折的临床效果显著,可促进术后功能恢复,缓解疼痛,预防并发症的发生,临床应用价值高。Objective To investigate the clinical effect of proximal femoral nail anti-rotation(PFNA)in the treatment of intertrochanteric fractures of the elderly.Methods A retrospective selection of 60 elderly patients with intertrochanteric fractures admitted to our hospital from July 2018 to December 2019 was made.According to the different research methods,they were divided into control group and study group,with 30 cases in each group.The control group was treated with locking compression plate(LCP),the study group was treated with PFNA internal fixation.The operation time,hospitalization time,clinical fracture healing time,intraoperative blood loss,Harris hip function,visual analogue scale(VAS)score and complications were observed in the two groups.Results The operation time,hospitalization time and clinical fracture healing time of the study group were shorter than those in the control group,and the amount of intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05).At 2 weeks and 1 month after surgery,the Harris hip function score of the study group was higher than that of the control group,the difference was statistically significant(P<0.05);the postoperative VAS score in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no complication in the study group,and the total complication rate in the control group was 6.67%,there was no significant difference between the two groups(P>0.05).Conclusion PFNA internal fixation has a significant clinical effect in the treatment of femoral intertrochanteric fractures in the elderly,which can promote postoperative functional recovery,relieve pain,prevent complications,and have high clinical application value.
