摘  要:目的:了解包头市14岁以下儿童超声骨密度水平,构建西部地区骨密度数据库,评价和分析超声骨密度的正常值范围、变化规律及其影响因素。方法:按照随机数字表随机抽取2018年11月-2019年8月来我院儿童保健科进行体检的0~14岁健康儿童,共1968例。测量研究对象的一般数据、超声骨密度、血清25(OH)D、血清钙水平。按年龄、性别、血清25(OH)D水平、血清钙水平分组,分析超声骨密度的分布情况及影响因素。结果:0~14岁儿童超声骨密度平均水平(3145.36±281.69)m/s,不足率30.92%。超声骨密度平均水平随月龄变化逐渐增加,0~12月龄时骨密度增速最快,之后增速逐渐放缓。各年龄组间进行多重比较,各组间骨密度不相同,差异有统计学意义(P<0.05)。血清维生素D正常组骨密度值优于维生素D不足组和缺乏组,差异有统计学意义(P<0.05)。血钙缺乏组、正常组、过量组超声骨密度各组间差异无统计学意义。在多因素分析中,年龄和血清维生素D是影响儿童超声骨密度水平的主要因素。结论:本地区0~14岁儿童超声骨密度水平较全国健康儿童骨密度平均水平略有偏低,年龄和血清维生素D是影响儿童超声骨密度水平的主要因素,3岁前是骨密度增长的关键时期,故出生后即应注意维生素D和钙的合理补充以期提高骨密度水平。Objective:To understand the ultrasonic bone mineral density of children under 14 years old in Baotou City,to construct a bone mineral density database in western China,and to evaluate and analyze the normal range,variation law and influencing factors of ultrasonic bone mineral density.Methods:According to the random number table,a total of 1968 healthy children aged 0-14 who were admitted to the Children's Health Department of our hospital from November 2018 to August 2019 were selected.General data of the subjects,ultrasound bone density,serum 25(OH)D,serum calcium levels were measured.The distribution of ultrasound bone mineral density and its influencing factors were analyzed by age,sex,serum 25(OH)D and serum calcium.Results:The average level of ultrasound bone mineral density for children aged 0-14 years was(3145.36±281.69)m/s,and the deficiency rate was 30.92%.The average level of ultrasonic bone density gradually increased with the change of age,and the growth rate peak of bone density was 0-12 months,and then the growth rate gradually slowed down.Multiple comparisons were made between age groups.The bone mineral density was not the same between the groups,and the difference was statistically significant.The bone mineral density of the normal group of serum vitamin D was better than that of the vitamin D deficiency group and the lack group,and the difference was statistically significant.There was no significant difference in the bone mineral density between the blood calcium deficiency group,the normal group and the excess group.In multivariate analysis,age and serum vitamin D were the main factors affecting children's ultrasound bone mineral density.Conclusion:Ultrasound bone mineral density in children aged 0-14 years in this region is slightly lower than the average level of bone mineral density in healthy children in China.Age and serum vitamin D are the main factors affecting children's ultrasound bone mineral density.Before 3 years old is the key period of bone mineral density growth.Therefore,atte
