腹腔镜下疝囊高位结扎联合脐正中襞修补术与传统手术对小儿腹股沟斜疝术后复发率的影响

摘　　要：目的观察腹腔镜下疝囊高位结扎联合脐正中襞修补术与传统手术对小儿腹股沟斜疝术后复发率的影响。方法采用回顾性研究方法,选取2017年6月至2019年12月亳州市人民医院收治的120例腹股沟斜疝患儿,根据手术方法的不同将其分为研究组(n=62)和对照组(n=58),研究组患儿行腹腔镜下疝囊高位结扎联合脐正中襞修补术,对照组患儿行传统手术。观察两组患儿的相关手术指标、术后疼痛情况及术后相关并发症发生情况及复发情况。结果研究组患儿的手术时间(25.42±3.92) min、术中出血量(20.09±1.82) ml和住院时间(2.15±0.17) d均明显少于对照组患儿(31.79±3.38 min、29.13±3.26 ml和3.05±0.21 d),差异有统计学意义(P <0.05)。研究组患儿的疼痛程度优良率(96.77%)明显高于对照组(82.76%),差异有统计学意义(P <0.05)。研究组患儿的总并发症发生率(3.33%)明显低于对照组(17.24%),差异具有统计学意义(P <0.05)。研究组患儿的复发率(0.00%)明显低于对照组(8.62%),差异具有统计学意义(P <0.05)。结论腹腔镜下疝囊高位结扎联合脐正中襞修补术可明显降低腹股沟斜疝患儿的疼痛程度,减少相关并发症的发生情况,且可有效预防术后复发,值得临床推广应用。Objective To observe the effect of laparoscopic high ligation of hernia sac combined with median umbilical fold repair and traditional surgery on the recurrence rate of pediatric indirect inguinal hernia.Methods A retrospective study was performed,and 120 children with indirect inguinal hernia treated in Bozhou People’s Hospital from June 2017 to December 2019 were selected and divided into study group (n=62)and control group (n=58) according to different surgical methods.The patients in the study group used laparoscopic high ligation of hernia sac combined with median umbilical fold repair,and the patients in the control group used traditional surgery.The surgery related indexes,postoperative pain,postoperative complications and recurrence rate of the two groups were observed.Results The operation time (25.42±3.92) min,intraoperative bleeding volume (20.09±1.82) ml and hospitalization time (2.15±0.17) d of the children in the study group were less than those in the control group[(31.79±3.38) min,(29.13±3.26) ml and (3.05±0.21) d],and the differences were statistically significant (P<0.05).The excellent rate of pain in the study group was significantly higher than that in the control group (96.77%vs.82.76%),and the difference was statistically significant (P<0.05).The incidence of total complications in the study group was significantly lower than that in the control group (3.33%vs.17.24%),and the difference was statistically significant (P<0.05).The recurrence rate in the study group was significantly lower than that in the control group (0.00%vs.8.62%),and the difference was statistically significant (P<0.05).Conclusion Laparoscopic high ligation of hernia sac combined with median umbilical fold repair can significantly reduce the pain level of children with indirect inguinal hernia,reduce the incidence of related complications,and effectively prevent postoperative recurrence,which is worthy of clinical promotion.
