摘　　要：以上海市华东医院的实践为例,介绍了医务社会工作在团队整合、流程整合、服务整合、平台整合和组织整合等方面的路径探索;最后总结了医务社会工作介入整合型健康服务的路径特点和社会意义。Taking the Huadong Hospital of Shanghai as an example,this paper introduces the path exploration of medical social work in the aspects of team integration,process integration,service integration,platform integration and organization integration.Finally,the paper summarizes the characteristics of medical social work and the social significance of integrated health service.
