地屈孕酮及阿司匹林联合用药治疗习惯性流产效果观察

摘　　要：目的探究地屈孕酮及阿司匹林联合用药治疗习惯性流产效果。方法研究纳入共计97例习惯性流产患者,均由郑州大学第三附属医院2017年7月—2018年7月期间收治,采取随机数字表法将患者分为两组,对照组患者(48例)采取地屈孕酮治疗,观察组患者(49例)采取地屈孕酮+阿司匹林治疗,比较两组患者治疗后妊娠结局、治疗前后凝血功能指标变化情况及治疗后两组患者免疫功能指标情况、药物不良反应发生率。结果观察组患者妊娠存活率(89.79%)高于对照组(70.83%),流产率(10.20%)低于对照组(29.17%),差异有统计学意义(P<0.05);观察组患者足月妊娠率(90.91%)高于对照组(73.53%),早产率(9.09%)低于对照组(26.47%),差异有统计学意义(P<0.05);治疗前,两组患者凝血功能指标水平相当,治疗后各组改善,观察组优于对照组,差异有统计学意义(P<0.05);观察组患者治疗后免疫功能指标优于对照组,差异有统计学意义(P<0.05);观察组患者不良反应率(4.08%)与对照组相当(4.17%),差异无统计学意义(P>0.05)。结论地屈孕酮及阿司匹林联合用药可提升习惯性流产妊娠存活率及胎儿足月妊娠率,患者凝血功能及免疫功能改善,不良反应少,安全性高,值得推广及应用。Objective To explore the effect of diloprogesterone combined with aspirin in the treatment of habitual abortion.Methods Study included a total of 97 patients with recurrent miscarriage from July,2017 to July,2018.They were divided into two groups by random sampling method,including control group(48 cases)with flexion of progesterone therapy,and observation group(49 cases)with bend progesterone plus aspirin treatment.After treatment,pregnancy outcomes,and the changes of blood co agulation function indexes before and after treatment in the two groups,and immune function index and the incidence of adverse drug reactions of two groups were compared.Results Pregnancy survival rate(89.79%)in the observation group was higher than that in the control group(70.83%),and abortion rate(10.20%)was lower than that in the control group(29.17%),(P<0.05).The full-term pregnancy rate(90.91%)in the observation group was higher than that in the control group(73.53%),and the pre term delivery rate(9.09%)was lower than that in the control group(26.47%),(P<0.05).Before treatment,the blood coagulation function indexes of the two groups were similar,and each group improved after treatment.The observation group was superior to the control group(P<0.05).After treatment,the immune function index of the observation group was better than that of the con trol group(P<0.05).The adverse reaction rate(4.08%)in the observation group was the same as that in the control group(4.17%),and they were both lower than that in the control group(P>0.05).Conclusion The combination of desdrogesterone and aspirin can improve the pregnancy survival rate of habitual abortion and the full-term pregnancy rate of fetus.The coagulation function and immune function of patients are improved with fewer adverse reactions and high safety.It is worth popularizing and applying.
