超声弹性成像评估对桥本甲状腺炎背景下甲状腺良恶性结节鉴别诊断效能分析

摘　　要：目的分析超声弹性成像评估用于HT背景下甲状腺良恶性结节鉴别诊断的效能,并与常规超声进行对比。方法选取我院因桥本甲状腺炎收治的患者116例,患者入院后均接受常规超声方法及超声弹性成像方法的检查。比较两种方法对于HT背景下甲状腺结节的诊断效能。结果恶性结节的形态不规则、边界模糊、低回声、微钙化、阻力指数RI>0.7、血流讯号Ⅱ级及以上等指标的占比明显高于良性结节(P<0.05);甲状腺恶性结节的弹性评分、应变率比值均明显高于良性结节(P<0.05);超声弹性成像方法诊断HT背景下甲状腺结节时,其灵敏度、准确度、阴性预测值明显优于常规超声方法(P<0.05)。结论在桥本甲状腺炎背景下甲状腺良恶性结节的鉴别诊断中,超声弹性成像评估相较于常规超声检查具有更高的灵敏度及准确度,有利于早期行有效干预措施、尽可能规避结节恶化的危险。Objective To analyze the efficacy of ultrasound elastography to assess the differential diagnosis of benign and malignant nodules in the thyroid gland in the context of HT and to compare it with conventional ultrasound.Methods The study population was selected from 116 patients admitted to our hospital for Hashimoto′s thyroiditis from,and all patients underwent routine ultrasound method and ultrasound elastic imaging method after admission.The diagnostic efficacy of the two methods was compwed for thyroid nodules in the HT background.Results Malignant nodules with irregular morphology,blurred boundaries,low echo,microcalcification,resistance index RI higher than 0.7,blood flow signal grade II and above were significantly higher than benign nodules(P<0.05),Elasticity score and strain ratio of thyroid malignant nodules were significantly higher than benign nodules(P<0.05),The sensitivity,accuracy and negative predictive value of ultrasound elastic imaging method for diagnosis of thyroid nodules in HT background were significantly better than conventional ultrasound method(P<0.05).Conclusions In the differential diagnosis of benign malignant nodules of the thyroid gland in the context of Hashimoto′s thyroiditis,ultrasound elastography assessment has a higher sensitivity and accuracy than conventional ultrasound,facilitating early and effective intervention and minimizing the risk of nodal progression.
