摘　　要：目的探讨肺部超声检查肺湿变区域数在评估急性呼吸窘迫综合征严重程度及预后判断中的作用。方法选取因呼吸衰竭入住我院重症监护室患者134例。其中诊断急性呼吸窘迫综合征的65例为实验组,另69例为对照组。收集两组患者的年龄、ApacheⅡ评分,均进行肺部超声检查记录肺湿变区域数,收集患者预后资料包括ICU死亡率、住院死亡率、住ICU天数、总住院天数等数据,对两组数据进行对比分析。结果急性呼吸窘迫综合征组患者的肺超声检查肺湿变区域数高于非呼吸窘迫综合征组的患者[6(2~12)与4(0~8),P<0.05],且两组ApacheⅡ评分、ICU死亡率、住院死亡率、住ICU天数、总住院天数差异有统计学意义;肺超声检查肺湿变区域数在不同程度呼吸窘迫综合征患者存在差异(P<0.05),与严重程度呈正相关。结论肺超声检查肺部湿变区域数在评估ARDS患者严重程度及预后上有正相关性,可以作为急性呼吸窘迫综合征的辅助评价手段。Objective To explore the role of lung ultrasound in assessing the severity and prognosis in patients with acute respiratory distress syndrome.Methods This was a study in intensive care unit(ICU)of Guangdong second province general hospital.Patients who required intensive care for respiratory failure on ICU admission from were included.65 cases were diagnosed as acute respiratory distress syndrome in experimental group,and 69 cases were control group.We collected the ICU and hospital mortality,length of stay and ApacheⅡ,analysis the data of two groups.Results Compared with the patients withont acute respiratory distress syndrome,the patients with acute respiratory distress syndrome,the wet changes lung regions numbers were higher[6(2~12)or 4(0~8),P<0.05].ICU and hospital mortality,length of stay and ApacheⅡwere higher too.The wet changes lung regions numbers were positively associated with the severity of acute respiratory distress syndrome.Conclusions The numbers of lung regions with wet changes has a positive correlation between the severity and prognosis of patients with ARDS,and can be used as an evaluation method for evaluation acute respiratory distress syndrome.
