肺超声与HRCT诊断结缔组织病相关间质性肺病的对比分析

摘　　要：目的探讨肺超声在诊断结缔组织病相关间质性肺病时与HRCT的一致性。方法选取在我院风湿免疫科病区收治患有结缔组织病的60例患者。所有患者均进行HRCT检查和LUS检查,以HRCT为金标准,对比两种检查结果,并对检查结果进行一致性分析。结果肺超声诊断CTD-ILD的灵敏度、特异度、阳性预测值和阴性预测值分别为97.9%、83.3%、95.9%和90.9%。McNemar检验结果显示两种检查方法诊断结果无统计学差异(P=1.000),一致性检验结果显示两种检查方法诊断结果一致性Kappa值为0.839。结论LUS诊断CTD-ILD有很高的准确度和诊断效能,且与HRCT有较好的一致性。Objective To investigate the consistency of lung ultrasound with HRCT in the diagnosis of connective tissue disease associated interstitial lung disease(CTD-ILD).Methods 60 patients with CTD were enrolled in the rheumatology department of our hospital.All patients underwent HRCT and LUS.HRCT was used as the gold standard,the results of the two examinations were compared,and their consistency were analyzed.Results The sensitivity,specificity,positive and negative predictive value of LUS were 97.9%,83.3%,95.9%and 90.9%,respectively.There was no statistical difference in the diagnostic results of the two methods(P=1.000),and the Kappa value of the diagnostic results was 0.839.Conclusions LUS has high accuracy and diagnostic efficiency in the diagnosis of CTD-ILD,and has good consistency with HRCT.
