常规超声假阴性与钼靶X线假阴性乳腺癌的临床病理特征比较

摘　　要：目的旨在通过比较常规超声假阴性乳腺癌与钼靶X线假阴性乳腺癌两类病例之间的临床及病理特征,进一步探讨超声及钼靶对于早期乳腺癌的诊断价值和差异性。方法回顾性分析于我院70例超声假阴性乳腺癌及93例钼靶假阴性乳腺癌病例,以术后病理作为金标准,比较两组病例在年龄、肿瘤大小、肿瘤病理类型、病理组织学分级、淋巴结转移情况、ER、PR、Ki-67表达水平、her-2等各项指标的差异。结果和钼靶假阴性乳腺癌相比,超声假阴性乳腺癌肿瘤比例较小(1.45 cm vs 1.77cm,P=0.018),原位癌比例较高(37.1%vs 12.9%,P<0.01),高级别乳腺癌比例较低(14.9%vs 20.4%,P<0.01),PR阳性率比例较低(53.7%vs 69.2%,P<0.01),Ki-67高表达比例较低(58.5%vs 68.8%,P=0.02,),her-2阳性比例较高(24.4%vs 14.9%,P=0.03)。在年龄、ER表达水平上两者的差异不具有显著统计学意义(P>0.05)。结论常规超声较钼靶X线更容易漏诊导管原位癌,以及体积较小、分化程度较高、腋下淋巴结无转移的乳腺癌。Objective To compare the clinical and pathological characteristics between ultrasound(US)false negative breast cancer and mammography(MG)false negative breast cancer,and to further explore the value and difference of US and MG in the diagnosis of early breast cancer.Methods From 70 cases of US false negative breast cancer and 93 cases of MG negative breast cancer were retrospectively analyzed.The clinicopathological features were compared with the gold standard of pathology.Results Compared with MG false negative breast cancer,US false negative breast cancer had smaller tumors(1.45 cm vs 1.77 cm,P=0.018),more in situ cancer(37.1%vs 12.9%,P<0.01),lower grade(14.9%vs 20.4%,P<0.01)、lower PR positive(53.7%vs 69.2%,P<0.01),lower Ki-67(58.5%vs 68.8%,P=0.02),higher her-2 positive(24.4%vs 14.9%,P=0.03).There was no significant difference in age and ER expression between the two groups(P>0.05).Conclusions Compared with MG false negative breast cancer,US false negative breast cancer has smaller size,more DCIS,lower grade,less lymph node metastasis.
