摘　　要：目的评价昌平区146名新生儿全程接种10μg重组乙肝疫苗(hepatitis B vaccine,HepB)5年后的免疫效果及其影响因素。方法应用队列研究的方法,选用10μg重组汉逊酵母HepB,按照"0-1-6"月龄免疫程序完成3剂次初免的新生儿,并在接种第3剂次后和5年后分别采集静脉血,用同样方法检测,计算抗体阳性率和几何平均滴度(geometric mean titer,GMT)。通过logistic回归分析抗体阳性率的影响因素。结果随访儿童感染率为0,5年抗-HBs(hepatitis B surface antibody,抗-HBs)阳性率为79.45%,GMT为37.41 mIU/mL,5年抗-HBs水平与初免抗-HBs水平呈正相关(r=0.406,P<0.01)。多因素logistic回归分析显示,调查对象5年抗-HBs抗体阳性率呈初免高应答儿童高于中低应答儿童(OR=2.24,95%CI:1.07~4.65)、出生6个月时能坐稳儿童高于不能坐稳者(OR=3.10,95%CI:1.19~8.09)。结论新生儿接种10μg重组HepB 5年后抗-HBs依然有较高水平。Objective To evaluate the immunization effectiveness and influencing factors of Hepatitis B vaccine(HepB)of 10μg for 5 years among 146 neonates in Changping district after whole-course vaccination.Methods Using the method of cohort study,three initial doses of recombinant Henson's yeast HepB of 10μg was inoculated for newborns according to the"0-1-6"month-old immunization program.Venous blood was collected after the third dose and in 5 years,and the samples were tested by the same method.The influencing factors of antibody positive rate were analyzed by logistic regression.Results The infection rate of the children was 0,and the positive rate of hepatitis B surface antibody(HbsAb)for 5 years was 79.45%with a GMT value of 37.41 mIU/ml.The level of HbsAb for 5 years was positively correlated with the level of HbsAb at primary treatment(r=0.406,P<0.01).Multivariate Logistic regression analysis showed that the positive rate of HbsAb in five years among children with high response was higher than that among the children with low response(OR=2.24,95%CI:1.07-4.65)and was higher in the children who were stable at 6 months of age than that in the children who were not(OR=3.10,95%CI:1.19-8.09).Conclusions Nneonatal immunized with HepB of 10μg showd high antibody levels after 5 years.
