加味忍冬藤汤对尿酸性肾病湿热内蕴证患者血尿酸、血β2微球蛋白的影响

摘　　要：目的:观察加味忍冬藤汤对尿酸性肾病湿热内蕴证患者血尿酸、血β2微球蛋白的影响。方法:选择2018年1月~2019年3月我院的湿热内蕴证尿酸性肾病患者共60例,随机分为治疗组和对照组各30例。对照组予常规治疗及口服别嘌醇片,在此基础上,治疗组予加味忍冬藤汤口服,观察两组患者疗效,中医症候改善情况、监测治疗前后肾功及血、尿β2微球蛋白水平。结果:治疗后,治疗组疗效较对照组明显提高,患者四肢沉重,关节灼热肿痛以及颜面或下肢浮肿明显减轻,治疗组血肌酐、尿素氮、血尿酸、24h尿蛋白定量及肾小球滤过率等肾功能较治疗组明显改善(P<0.05或0.01)。治疗组尿β2-微球蛋白及血β2-微球蛋白较对照组明显降低,(P<0.05或0.01)。结论:加味忍冬藤汤在治疗尿酸性肾病湿热内蕴证患者时,可显著降低血尿酸、血β2微球蛋白,改善患者肾功。Objective:To observe the effect of Modified Lonicerae Decoction on blood uric acid and blood β2 microglobulin for uric acid nephropathy patients with dampness-heat type.Methods:A total of 60 patients with uric acid nephropathy were enrolled in our hospital from January 2018 to March 2019.They were randomly divided into treatment group and controlled group.The controlled group received conventional treatment and oral allopurinol tablets.On this basis,the treatment group was treated with Modified Lonicerae Decoction.The effect of the two groups were observed.The improvement of TCM symptoms,renal function before and after treatment,and blood and urine β2 microglobulin level were recorded.Results:After treatment,the curative effect of the treatment group was significantly higher than that of the controlled group.The patients with heavy limbs,joint burning and swelling,and facial or lower extremity edema in the treatment group had significantly improved.The creatinine,urea nitrogen,uric acid,24 h urine protein and glomerular filtration rate were better in the treatment group(P<0.05 or 0.01).Urinary β2-microglobulin and blood β2-microglobulin were significantly lower in the treatment group than in the controlled group(P<0.05 or 0.01).Conclusion:Modified Lonicerae Decoction can significantly reduce blood uric acid and blood β2 microglobulin and improve renal function in uric acid nephropathy patients with dampness-heat type.
