摘 要:目的研究急性颅脑损伤患者血清中血清胱抑素C(cystatin C,Cys-C)、组织基质金属蛋白酶抑制因子-2(TIMP-2)水平与患者急性肾功能损伤发生及病情的关系。方法选择2017年12月-2018年12月本院收治的120例重症颅脑损伤患者作为研究对象,所有入院后均检测血清中Cys-C、TIMP-2的表达,随访患者住院期间是否发生AKI,对比不同病情AKI患者临床特点及血清中Cys-C、TIMP-2,探讨患者发生AKI的危险因素。结果 AKI组Cys-C、TIMP-2水平明显高于对照组(P <0.05),且随着病情加重Cys-C、TIMP-2水平逐渐增高;AKI组轻度、中度、重度患者48 h血清Cys-C、TIMP-2水平明显高于0 h、24 h(P <0.05),且随着时间的进展Cys-C、TIMP-2水平越来越高。甘露醇使用、基础Scr、心功能衰竭、高血压是患者出现AKI的独立风险因素(P <0.05)。结论血清中Cys-C、TIMP-2水平与颅脑损伤患者急性肾损伤病情及预后相关。Objective To study the relationship between serum levels of Cys-C,TIMP-2 and the occurrence and severity of acute renal injury in patients with acute craniocerebral injury.Methods A total of 120 patients with severe craniocerebral injury admitted to our hospital from December 2017 to December 2018 were selected as the study subjects.The serum levels of Cys-C and TIMP-2 were detected after admission.The patients were followed up for AKI during hospitalization.The clinical characteristics and serum Cys-C and TIMP-2 of AKI patients with different conditions were compared to explore the risk factors of AKI in patients.Results The levels of Cys-C and TIMP-2 in AKI group were significantly higher than those in control group(P<0.05),and the levels of Cys-C and TIMP-2 gradually increased with the aggravation of the disease.The serum levels of Cys-C and TIMP-2 in patients with mild,moderate and severe AKI at 48 h were significantly higher than those at 0 h and 24 h(P<0.05),and the levels of CYS-C and TIMP-2 became higher with time.Mannitol use,basal Scr,heart failure and hypertension were independent risk factors for AKI(P<0.05).Conclusion Serum levels of Cys-C and TIMP-2 are correlated with the severity and prognosis of acute renal injury in patients with craniocerebral injury.
