当归芍药散对心力衰竭患者血Cysc和β2-MG水平影响及其机制的研究

摘　　要：目的:探究当归芍药散对心力衰竭患者血Cysc和β2-MG水平影响并分析其机制。方法:搜集整理2014年1月至2019年12月在南阳理工学院附属医院收治的的心力衰竭门诊、住院患者的相关病历资料进行回顾性分析,将使用单纯西医治疗的患者设为对照组,使用当归芍药散治疗的患者设为治疗组。对两组患者治疗前后的血Cysc和β2-MG水平变化进行统计分析,比较其差异性。并分析治疗组Cysc和β2-MG水平变化与中医症状积分、心脏超声检查有关指标的相关性,探究相关机制。结果:治疗组与对照组心力衰竭患者治疗后28天血Cysc、β2-MG水平与治疗前比较均有降低,并且Cysc的变化较β2-MG灵敏,治疗后的14天即有明显改善,治疗组更明显,差异经统计学处理(p<0.05);使用当归芍药散治疗的治疗组患者治疗后28天血Cysc、β2-MG降低与中医症状积分降低一致(p<0.05);治疗后28天血Cysc、β2-MG降低与心脏心脏收缩、舒张功能的超声数值变化一致,LVEDV、LVESV、E/A比值降低,SV、LVEF升高,(p<0.05)结论:当归芍药散可降低心力衰竭患者血Cysc和β2-MG水平,改善其临床症状及心脏收缩、舒张功能;Cysc和β2-MG水平变化与中医症状积分、心脏超声数值变化一致,血Cysc更敏感客观。其机制与当归芍药散调理肝脾,养血活血利水有关。Objective:To explore the effects of danggui shaoyao powder on blood levels of Cysc and beta β2-MG in patients with heart failure and analyze its mechanism. Methods: To collect and collate the related medical records of out-patients and inpatients with heart failure from January 2014 to December 2019 in the Affiliated Hospital of Nanyang Institute of Technology,and make a retrospective analysis,the patients treated with Danggui Shaoyao powder were divided into treatment group. The changes of serum CYSC and β2-MG levels in the two groups before and after treatment were statistically analyzed and compared. The changes of CYSC and β2-MG levels in the treatment group were analyzed,and the correlation between them and TCM Symptom score and cardiac ultrasound was explored. Results: Compared with the control group,the level of CYSC and β2-MG in the treatment group and the Control Group decreased 28 days after treatment,and the change of CYSC was more sensitive than that of β2-MG. After 14 days of treatment,the level of CYSC was improved obviously,especially in the treatment group,the decrease of serum CYSC and β2-MG in the treatment group was consistent with the decrease of TCM symptom score(p< 0.05) At 28 days after treatment,the decrease of CYSC and β2-MG in blood was consistent with the changes of cardiac systolic and diastolic function. The ratios of LVEDV,LVESV and e/A were decreased,and the ratios of SV and LVEF were increased(p<0.05). Conclusion: Danggui Shaoyao San can reduce the level of serum CYSC and β2-MG in patients with heart failure and improve their clinical symptoms and cardiac systolic and diastolic function, blood Cysc is more sensitive and objective. Its mechanism is related to regulating liver and spleen,nourishing blood,promoting blood circulation and promoting diuresis.
