正颌术后颌间牵引对患者营养状况的影响

摘　　要：目的观察正颌术后颌间牵引对患者营养状况的影响。方法采集2016年6月-2017年6月本院口腔科收治的32例正颌术后颌间牵引患者的临床与随访资料,统计分析牵引前后及随访时本组患者颌间功能情况、症状自评量表(SCL-90)评分、血红蛋白(hemoglobin,Hb)、白蛋白(Albumin,ALB)、血清总蛋白(Total se⁃rum protein,TP)及体质量指数(Body mass index,BMI)和体重变化。结果与牵引前相较,本组32例患者牵引后、随访时咀嚼功能评分明显升高,颌面美观、发音、关节不适评分、Hb、ALB、TP明显降低(P<0.05);随访时咀嚼功能评分较牵引后略高,颌面美观、发音、关节不适评分较牵引后略低,差异不显著(P>0.05),但随访时本组患者Hb、ALB、TP较牵引后明显低(P<0.05),此外本组患者随访时的BMI与体重较牵引前明显降低(P<0.05)。结论正颌术后颌间牵引会导致患者营养状况不佳。Objective To observe the effect of intermaxillary traction on the nutritional status of the cases after orthognathic surgery.Methods The clinical data and follow-up data of 32 cases undergoing intermaxillary traction after orthognathic surgery during the period from June,2016 to June,2017 were collected,and a statistical analysis was made to the intermaxillary function,Symptom Check List-90(SCL-90)scores,hemoglobin(Hb)level,albumin(ALB)level,total serum protein(TP)level,body mass index(BMI)and weight of the cases before traction,after traction and during follow-up visit.Results The SCL-90 scores of chewing function were significantly higher and the SCL-90 scores of maxillofacial aesthetics,pronunciation and joint discomfort were significantly lower after traction and during follow-up visit than those before traction(P<0.05);the levels of Hb,ALB and TP of the cases after traction and during follow-up visit were significantly lower than those before traction(P<0.05);the SCL-90 scores of chewing function were slightly higher and the SCL-90 scores of maxillofacial aesthetics,pronunciation and joint discomfort were slightly lower during follow-up visit than those after traction(P>0.05);the levels of Hb,ALB and TP during follow-up were significantly lower than those after traction(P<0.05);the BMI and weight of the cases during follow-up were significantly lower than those before traction(P<0.05).Conclusions Intermaxillary traction may lead to poor nutritional status in cases after orthognathic surgery.
