二甲双胍对118例PCOS患者体外受精-胚胎移植妊娠结局的影响

摘　　要：目的观察使用二甲双胍对多囊卵巢患者进行干预的临床效果。方法选取2017年1月-2018年12月我院收治的118例PCOS患者,按随机数字表法分为观察组与二甲双胍治疗组,分析治疗前后两组BMI和内分泌激素、糖代谢指标以及临床与实验室各项参数的变化。结果两组患者在干预前各项指标包括年龄、BMI、血激素及血糖、胰岛素比较差异无统计学意义,干预后两组的BMI、LH、T等指标水平均降低,治疗组的血LH,T水平及FINS明显下降,差异有统计学意义(P<0.05);治疗组HCG日E2水平、流产率明显低于观察组,差异有统计学意义(P<0.05);OHSS发生率低于观察组(P>0.05),临床妊娠率高于对照组(P>0.05),出生率显著高于对照组(P<0.05)。结论在IVF助孕前或助孕中进行二甲双胍的干预性治疗是可以改善PCOS患者最终妊娠结局的,二甲双胍对接受IVF的PCOS患者是安全有益的用药,可以改善患者的妊娠结局。Objective To Observe clinical effect of metformin on the treatment of polycystic ovary.Methods A total of 118 PCOS patients admitted to our hospital from January 2017 to December 2018 were selected and divided into the observation group and the metformin treatment group according to the random number table method.Changes in BMI,endocrine hormones,glucose metabolism indexes and clinical and laboratory parameters of the two groups before and after treatment were analyzed.Results Two groups of patients in the intervention before the indicators include age,BMI,blood hormone insulin and blood sugar,there was no statistically significant difference comparison,the two groups after the intervention of BMI,LH,T levels are reduced,the treatment group's blood LH and T levels and FINS significantly decreased,the difference was statistically significant(P<0.05),treatment group HCG day E2 levels,abortion rates significantly lower than the observation group;The difference was statistically significant(P<0.05),the incidence of OHSS was lower than that of the observation group(P>0.05),the clinical pregnancy rate was higher than that of the control group(P>0.05),and the birth rate was significantly higher than that of the control group(P<0.05).Conclusions Metformin intervention treatment before IVF or during can improve the final pregnancy outcome of PCOS patients.Metformin is a safe and beneficial drug for PCOS patients receiving IVF,which can improve the pregnancy outcome of patients.
