不同强度华法林与达比加群酯在老年心房颤动合并高血压患者抗凝治疗中的应用

摘　　要：目的探讨不同强度华法林与达比加群酯在高龄心房颤动合并高血压患者抗凝治疗中的疗效及安全性。方法入选确诊为房颤合并高血压患者120例为研究对象,随机分为低强度华法林组、高强度华法林组,达比加群酯组,每组各40例,观察12个月,比较患者血栓检出率及血栓、出血事件发生率。结果治疗12周、24周、48周时,3组血栓检出率比较差异均无统计学意义(P>0.05);低强度华法林组、高强度华法林组、达比加群酯组血栓栓塞事件发生率差异无统计学意义(χ^2=1.013,P=0.314);与高强度华法林组比较,低强度华法林组、达比加群酯组出血事件发生率均较低(χ^2=4.267,P=0.039;χ^2=7.168,P=0.007)。结论低强度华法林与达比加群酯抗凝在老年心房颤动合并高血压患者抗凝治疗中疗效确切,安全性高。Objective To study the efficacy and safety of Warfarin in different intensity and Dabigatran etexilate for the elderly patients with Atrial Fibrillation and Hypertension.Methods A total of 120 elderly patients with Atrial Fibrillation and Hypertension were randomly divided into low-intensity warfarin group,high-intensity warfarin group and Dabigatran etexilate group,with 40 cases in each group,following-up for 12 months.The rates of thrombus detection,thrombosis and incidence of bleeding events were compared.ResultsAt 12,24,and 48 weeks of the treatment,the detection rate of thromboembolism was no significant difference in the three groups(P>0.05);the incidence of thromboembolic events was no significant difference between the low intensity warfarin group,the high intensity warfarin group,and the dabijia group(χ^2=1.013,P=0.314);compared with thehigh strength warfarin group,low strength warfarin group and dabigatran group had lower incidence of bleeding events(χ^2=4.267,P=0.039;χ^2=7.168,P=0.007).Conclusion Low-intensity warfarin and dabiglitazone anticoagulation in the elderly patients with atrial fibrillation combined with hypertension is effective and safe.
