摘 要:目的研究芪贞降糖颗粒对2型糖尿病肾病(diabetic nephropathy,DN)大鼠肾功能的改善作用。方法采用高脂饲养和腹腔注射链尿佐菌素(STZ)相结合的方法建立DN大鼠模型,将模型成功的大鼠随机分为模型组、二甲双胍组(二甲双胍,180 mg/kg体质量)、芪贞降糖颗粒低、中、高剂量组(芪贞降糖颗粒,50、100、200 mg/kg体质量),每组10只,另取10只大鼠为正常组,模型组和正常组大鼠灌服等体积纯化水,各组大鼠连续给药8周。测定大鼠体质量和空腹血糖(FBG)、检测肌酐(SCr)、尿素氮(BUN)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)含量。采用酶联免疫吸附试剂盒测定各组大鼠肾脏炎性因子白介素1(IL-1)、白介素6(IL-6)和肿瘤坏死因子(TNF-α)的含量。HE染色观察肾组织病理变化,电镜观察肾小球足细胞超微结构变化。结果与模型组比较,二甲双胍组、芪贞降糖颗粒高、中、低剂量组大鼠给药后体质量均升高,大鼠血清TC、TG、LDL-C含量均降低,HDL-C含量升高,差异有统计学意义(P<0.05或P<0.01)。与二甲双胍组给药后相比,芪贞降糖颗低剂量组给药后大鼠FBG均升高,差异有统计学意义(P<0.01),芪贞降糖颗粒低、中剂量组大鼠血清SCr和BUN含量均升高,组织IL-1、IL-6和TNF-α含量均升高,差异有统计学意义(P<0.05或P<0.01)。结论芪贞降糖颗粒能够降低DN大鼠FBG、血脂含量、血清SCr和BUN含量以及炎性因子表达,其作用机制与增加肾组织细胞稳定,改善肾小球基底膜厚度和纤维化程度有关。Objective To study the effect of Qizhen Jiangtang Granule on renal function of diabetic nephropathy(DN)rats.Methods DN rat model was established by high-fat feeding and intraperitoneal injection of streptozotocin(STZ).The successful rats were randomly divided into model group,metformin group(180 mg/kg body weight),low,medium and high-dose Qizhen Jiangtang granule groups(50,100,200 mg/kg body weight),and another 10 rats were taken as the normal group.The rats in the model group and the normal group were given the same volume of purified water for 8 weeks.The body weight,fasting blood glucose(FBG),creatinine(SCR),urea nitrogen(BUN),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)were detected.The levels of IL-1,IL-6 and HDL-C were measured by ELISA kit.The pathological changes of renal tissue were observed by HE staining and the ultrastructural changes of glomerular podocyte were observed by electron microscope.Results Compared with the model group,the body weight of rats in metformin group and the low,medium and high dose of Qizhen Jiangtang granule groups were increased,serum TC,TG,LDL-C contents were decreased,and HDL-C content was increased(P<0.05 or P<0.01).Compared with metformin group,FBG in the low-dose group increased significantly(P<0.01),serum SCR and BUN levels in the low and medium-dose groups of Qizhen Jiangtang granule were increased,and the contents of IL-1,IL-6 and TNF-αin the tissues were increased(P<0.05 or P<0.01).Conclusion Qizhen Jiangtang granule can reduce FBG,blood lipid content,serum SCR and BUN contents and the expression of inflammatory factors in the DN rats.Its mechanism of action is related to increasing the stability of renal tissue cells,improving the thickness of glomerular basement membrane and the degree of fibrosis.
