小柴胡汤加减联合阿奇霉素治疗小儿支原体肺炎临床评价

摘　　要：目的探讨小柴胡汤加减联合阿奇霉素治疗小儿支原体肺炎的临床疗效及对患儿炎性因子水平和免疫功能的影响。方法选取医院2016年1月至2018年2月收治的支原体肺炎患儿90例,按随机双盲法分为治疗组和对照组,各45例。两组患儿均予退热、止咳、祛痰、平喘、氧疗等基础治疗,并静脉滴注注射用乳糖酸红霉素及口服阿奇霉素干湿悬剂,治疗组患儿加服小柴胡汤加减。两组均连续治疗7 d。结果治疗组总有效率为91.11%,明显高于对照组的73.33%(P<0.05);治疗后,治疗组患儿的退热时间、胸部X线摄片病灶吸收时间、咳嗽消失时间、住院时间均明显短于对照组,C反应蛋白、白细胞介素1β、降钙素原水平均明显低于对照组,免疫球蛋白A、免疫球蛋白G、免疫球蛋白M水平均明显高于对照组(P<0.05);治疗组与对照组不良反应发生率相当(17.78%比11.11%,P>0.05)。结论小柴胡汤加减联合阿奇霉素治疗小儿支原体肺炎,能快速改善患儿临床症状,降低炎性因子水平,改善免疫功能。Objective To study the clinical efficacy of Jiawei Xiaochaihu Decoction combined with azithromycin for children with mycoplasma pneumonia and its influence of inflammatory factors and immune functions.Methods A total of 90 children with mycoplasma pneumonia admitted to the hospital from January 2016 to February 2018 were selected and divided into the treatment group and control group according to random double blind method,45 cases in each group.Both groups were treated with basic therapies such as antipyretic,cough relieving,expectoration,anti-asthmatic,and oxygen therapy,as well as intravenous infusion of erythromycin lactobionate and oral azithromycin dry and wet suspension;the treatment group was added with Jiawei Xiaochaihu Decoction.Both groups were treated continuously for 7 d.Results The total effective rate of the treatment group was 91.11%,which was significantly higher than 73.33%in the control group(P<0.05);after treatment,the antipyretic time,chest X-ray absorption time,cough disappearance time,and hospital stay of the treatment group were significantly shorter than those in the control group;the levels of C-reactive protein(CRP),interleukin 1β(IL-1β)and procalcitonin(PCT)were significantly lower than those of the control group;the levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)were significantly higher than those of the control group(P<0.05);the incidence of adverse reactions in the treatment group and the control group was equivalent(17.78%vs.11.11%,P>0.05).Conclusion Jiawei Xiaochaihu Decoction combined with azithromycin for children with mycoplasma pneumonia can rapidly improve the clinical symptoms of children,reduce the level of inflammatory factors,and improve the immune function of children.
