远程心电监测在心房颤动射频消融术后早期复发监测中的应用

摘　　要：目的探讨远程心电监测对心房颤动射频消融术后患者随访中早期复发的监测作用。方法42例阵发性心房颤动患者接受环肺静脉射频消融术治疗房颤,术后3个月内采用心电事件记录器进行远程心电监测,同时分别在术后1周、1个月、3个月时进行门诊随访,使用常规心电图及24 h动态心电图检查。比较不同监测手段对房颤射频消融术后早期复发的检出情况。结果42例患者均完成3个月随访,早期复发19例,复发率为45.2%。其中,常规心电图检查检出6例(14.1%),动态心电图检出12例(28.6%),远程心电监测检出17例(40.5%);远程心电监测检出率显著高于常规心电图(P<0.05),动态心电图与远程心电监测间差异无统计学意义(P>0.05)。有症状时进行心电监测比无症状时心电监测房性心律失常的检出率更高(P<0.05)。结论与传统监测方法相比,远程心电监测方案能够有效监测房颤消融术后早期复发。有症状时及时进行心电监测可能提高术后房性心律失常的检出率。Objective To investigate the role of remote electrocardiogram monitoring in the early relapse of patients with atrial fibrillation after radiofrequency catheter ablation(RFCA).Methods Forty-two patients with paroxysmal atrial fibrillation who underwent RFCA in our hospital were enrolled in the study.The patients were followed up for 3 months and received 12-lead ECG and 24h Holter monitoring at 1w,1m,3m after FRCA.The early relapse status after RFCA was observed and compared among the different monitoring means.Results A total of 42 patients completed the 3-month follow up.And the atrial arrhythmia was detected in 19 patients,with relapse rate being 45.2%,in which,6 cases(14.1%)were detected by conventional ECG,and 12 cases(28.6%)were detected by dynamic ECG,and 17 cases(40.5%)were detected by remote ECG monitoring.The detection rate of remote ECG was significantly higher than that by conventional ECG,however,there was no significant difference between dynamic ECG and remote ECG monitoring(P>0.05).Moreover the detction rate in patients with clinical symptoms was significantly higher than that of patients without clinical symptoms.Conclusion As compared with traditional monitoring method,the remote electrocardiogram monitoring can effectively detect the early recurrence of patients with atrial fibrillation after RFCA,moreover,the timely electrocardiogram monitoring in patientts with clinical symptoms can improve the detection rate of atrial arrhythmia after RFCA.
