健康成年人斜方肌及相关穴位剪切波弹性成像特征研究

摘　　要：目的:分析健康成年人斜方肌及相关穴位剪切波弹性成像特征,为斜方肌劳损的诊断提供科学依据。方法:收集180例健康成年人为研究对象,男女各90例,均无临床症状,颈背部超声无异常发现。根据斜方肌劳损常规诊疗位置,选择3个穴位(颈百劳、肩中俞、肩井穴)作为观察点,应用声触诊组织量化(VTIQ)技术测得3个穴位处斜方肌的剪切波速度值,每个点测量3次,取平均值,测量分松弛状态及紧张状态。运用统计学软件比较不同年龄、不同性别、不同部位、不同侧别、不同状态斜方肌剪切波速度值的差异,分析斜方肌的弹性成像特征。结果:①不同年龄间斜方肌的剪切波速度值差异无统计学意义(P> 0.05)。②不同性别间同一穴位的剪切波速度值差异均无统计学意义(均P> 0.05)。③正常斜方肌左右侧对应部位的剪切波速度值差异无统计学意义(P> 0.05)。④紧张和松弛状态下斜方肌的剪切波速度值差异有统计学意义(P<0.001)。⑤颈百劳穴和肩井穴处剪切波速度值差异有统计学意义(P=0.001)。结论:斜方肌的正常剪切波速度值在不同位置、不同状态时有差异,与年龄、性别及左右习惯手无关系。剪切波弹性成像技术可较精确地反映斜方肌的硬度,为斜方肌劳损诊断提供科学依据。Objective:To analyze the characteristics of shear wave elastography of the trapezius and its acupoints in healthy adults and provide scientific basis for the diagnosis of trapezius strain.Methods:180 healthy adult subjects were recruited as study subjects.90 male and 90 female subjects who had no clinical symptoms and no abnormal findings in cervical and back ultrasound.According to the conventional diagnosis and treatment positions of trapezius strain,3 acupoints(Jingbailao,Jianzhongyu and Jianjing acupoint)were selected as observation points.Virtual touch tissue imaging quantification(VTIQ)was used to measure the shear wave velocity of trapezius at the 3 acupoints of the relaxation state and the tension state.The data at each point were measured for 3 times and the average value was taken.Statistical comparison was conducted to analyze the differences of shear wave velocity values of the trapezius at different ages,genders and sites,the same site on the left and right sides,and different states,so as to comprehensively analyze the elastography characteristics of the trapezius.Results:There was no significant difference in shear wave velocity between different ages,genders and the left and right side of normal trapezius(all P>0.05).The difference of shear wave velocity between tension and relaxation states was statistically significant(P<0.001).There was a statistically significant difference in the shear wave velocity between Jingbailao acupoint and Jianjing acupoint(P=0.001).Conclusions:The normal shear wave velocity of the trapezius varies in different positions and states,and it is not related to age,gender and left or right hand.The shear wave elastography can accurately reflect the trapezius hardness and provide scientific basis for the diagnosis of trapezius strain.
