摘　　要：目的:探讨背景抑制扩散加权成像(DWIBS)联合ADC值在鉴别宫颈癌盆腔淋巴结转移(LNM)与反应性增生淋巴结(RHP)中的应用价值。方法:回顾性分析经手术病理证实的45例宫颈癌的MRI与临床资料。以横断面为主要测定平面,测量LNM及RHP的DWIBS信号强度及ADC值。结果:45例共收集淋巴结50枚,其中,LNM 35枚,RHP 15枚。LNM的DWIBS信号强度为197.35±13.69,RHP为125.88±22.87,两者差异有统计学意义(t=4.309,P<0.01)。LNM的ADC值为(0.83±0.12)×10^(-3)mm^2/s,RHP为(1.18±0.14)×10^(-3) mm^2/s,两者差异有统计学意义(t=-3.857,P<0.01)。结论:DWIBS信号强度联合ADC值能够准确地鉴别LNM及RHP,对手术方案的制订及预后评估具有重要意义。Objective:To investigate the application value of background suppression DWI(DWIBS)combined with ADC value in distinguishing pelvic lymph node metastasis(LNM)of cervical cancer from reactive hyperplasia(RHP)based on MRI.Methods:MRI and clinicopathological data of 45 patients with cervical carcinoma were collected and retrospectively analyzed.DWIBS signal intensity and ADC values of LNM and RHP were measured with the cross section as the main measurement plane.Results:Among the 45 patients with 50 lymph nodes,35 LNM and 15 RHP were collected.The signal strength of LNM was 197.35±13.69,and the signal strength of RHP was 125.88±22.87.The difference of DWIBS signal intensity between the two groups was statistically significant(t=4.309,P<0.01).ADC value of LNM was(0.83±0.12)×10-3 mm2/s,ADC value of RHP was(1.18±0.14)×10-3 mm2/s,and the difference in ADC value between the two groups was statistically significant(t=-3.857,P<0.01).Conclusion:DWIBS signal intensity combined with ADC value can accurately distinguish LNM and RHP,which is of great significance for the formulation of clinical surgery and the prognosis of patients.
