摘  要:目的探讨早产儿血清维生素A(VA)的营养状况与支气管肺发育不良(BPD)之间相关性。方法回顾性分析2013年5月至2014年4月于该科住院的160例早产儿在不同VA水平时其用氧时间、BPD患病率有无差异。以病例对照方法,根据不同胎龄分层,比较BPD组与相匹配的非BPD组(对照组)患儿血清VA水平的差异。比较中、轻、重度BPD患儿之间血清VA水平的差异。结果160例早产儿根据VA的水平分为三组,维生素A缺乏(VAD)组用氧时间[192 h(98 h,306 h)]明显延长,与正常组[48 h(24 h,216 h)]及维生素A亚临床缺乏(SVAD)组[24 h(0 h,220 h)]比较,差异均有统计学意义(P<0.05),但三组间BPD患病率(VAD组:16.39%,SVAD组:13.33%,正常组:11.11%)比较,差异无统计学意义(P>0.05)。当34~37周胎龄时,BPD组血清VA水平[(0.2925±0.1444)μmol/L]低于对照组[(0.6144±0.1981)μmol/L],差异有统计学意义(P<0.05)。在23例患BPD的早产儿中,中重度BPD患儿血清VA水平[(0.3718±0.0404)μmol/L]低于轻度BPD[(0.3074±0.0319)μmol/L],但差异无统计学意义(P>0.05)。结论早产儿VA水平越低越容易出现氧依赖;患BPD的晚期(胎龄34~37周)早产儿VA水平更低。Objective To explore the correlation between the nutritional status of serum vitamin A(VA)and bronchopulmonary dysplasia(BPD)in preterm infants.Methods A retrospective analysis of 160 premature infants hospitalized in the department from May 2013 to April 2014 was performed at different VA levels to determine whether there was any difference in oxygen use time and BPD prevalence.Using a case-control method and stratifying according to different gestational ages,the differences in serum VA levels between the BPD group and the matched non-BPD group(control group)were compared.The differences in serum VA levels between children with moderate,mild,and severe BPD were compared.Results 160 premature infants were divided into three groups according to the level of VA.The oxygen duration[192 h(98 h,306 h)]in the vitamin A deficiency(VAD)group was significantly longer than that in the normal group[48 h(24 h,216 h)]and vitamin A subclinical deficiency(SVAD)group[24 h(0 h,220 h)],the differences were statistically significant(P<0.05),but the prevalence of BPD among the three groups(VAD group:16.39%,SVAD group:13.33%,normal group:11.11%),the difference was not statistically significant(P>0.05).When the gestational age was 34-37 weeks,the serum VA level in the BPD group[(0.2925±0.1444)μmol/L]was lower than that in the control group[(0.6144±0.1981)μmol/L],the difference was statistically significant(P<0.05).Among the 23 preterm infants with BPD,the serum VA level of children with severe BPD[(0.3718±0.0404)μmol/L]was lower than that of children with mild BPD[(0.3074±0.0319)μmol/L],but the difference was not statistically significant(P>0.05).Conclusion The lower VA level of premature infants,the more prone to oxygen dependence;the late preterm(gestational age 35-37 weeks)infants with BPD have lower VA levels.
