摘　　要：目的探讨流式细胞术(flow cytometry,FCM)检测HLA-B27抗原在葡萄膜炎诊断中的应用价值。方法采用荧光直接标记染色流式细胞技术检测患者外周血中CD3+淋巴细胞HLA-B27抗原的表达。结果 99例葡萄膜炎患者中,检出HLA-B27抗原阳性者38例,阳性率38.3%。HLA-B27抗原阳性患者的荧光强度高于HLA-B27抗原阴性者(P<0.01)。男、女性患者HLA-B27抗原阳性率差异无统计学意义(P>0.05)。38例HLAB27抗原阳性患者中有31例最终诊断为强直性脊柱炎(AS)伴发葡萄膜炎。结论 FCM检测外周血HLAB27抗原方便、快捷,结果准确可靠,可为临床诊断和鉴别诊断疾病及AAU的分型提供辅助依据。Objective To explore the application value of flow cytometry(FCM)in detecting human leucocyte antigen(HLA)B27 in diagnosis of uveitis. Methods The expression of CD3+ lymphocyte HLA-B27 antigen in peripheral blood of patients with uveitis was detected by fluorescence direct labeling of staining flow cytometry.Results Among 99 patients with uveitis,38 cases were positive for HLA-B27 antigen,with a positive rate of 38.3%. The fluorescence intensity of patients with positive HLA-B27 antigen was significantly higher than that of patients with negative HLA-B27 antigen(P<0.01). There was no statistical difference in a positive rate of HLA-B27 antigen between men and women(P>0.05). Of the 38 patients with positive HLA-B27 antigen,31 were diagnosed as ankylosing spondylitis(AS)with uveitis. Conclusion The detection of peripheral blood HLA-B27 antigen by FCM is convenient and fast,and the results are accurate and reliable,which can provide additional basis for clinical diagnosis and differential diagnosis of uveitis and classification of AAU.
