摘　　要：目的:研究应用一种根据上臂直径快速准确选择血压袖套的方法,以提高新生儿无创血压(NIBP)测量的准确性。方法:选取医院收治的48例新生儿,根据新生儿血压测量时使用血压袖套的不同,将根据患儿手臂周长选择血压袖套进行血压测量的纳入对照组,采用直径标识法选择血压袖套进行血压测量的纳入观察组。通过气囊宽度和(或)上臂周长比例值量化评估选择血压袖套,根据文献中的推荐比例,对比分析直径方法选择血压袖套的有效性,以及两组血压袖套差异对血压测量的影响。结果:在新生儿的日常血压检测中,对照组通过臂围选择的袖套有87.5%为偏小的袖套,会导致其血压测量结果偏高,而观察组通过上臂直径选择血压袖套的方法,正确选择合适血压袖套的占比为83.3%。对照组的血压测量结果有显著的偏高趋势,两组血压测量的收缩压、舒张压和平均动脉压比较,差异有统计学意义(t=3.047,t=3.126,t=2.975;P<0.01)。结论:新生儿NIBP测量时依据主观经验容易选择使用偏小的血压袖套,而通过直径选择的方法符合量化的客观标准,可协助临床医护人员有效获得新生儿正确的血压测量值。Objective: To recommend one kind of method that could fast and accurately choose oversleeve of blood pressure according to the diameter of the upper arm so as to improve the accuracy of the measurement of non-invasive blood pressure(NIBP) for neonate. Methods: 48 neonates who admitted to hospital were selected, and they were divided into control group that chose oversleeve of blood pressure as the perimeter of the arm of child patients and observation group that chose that as diameter identification method, according to different oversleeve of blood pressure when they received blood pressure measurement. The oversleeve of blood pressure was chosen by the quantitative evaluation of the ratio of width of air sac and(or) perimeter of the upper arm. And the effectiveness of choosing oversleeve of blood pressure as diameter method and the effects of the differences of oversleeve of blood pressure of two groups on the measurement of blood pressure were compared and analyzed. Results: In the measurement of routine blood pressure on neonate, 87.5% of the oversleeve that was chosen by arm circumference was smaller oversleeve which led to higher measurement results of blood pressure. While the percentage of choosing suitable oversleeve of blood pressure by the diameter of the upper arm was 83.3%. The measurement results of blood pressure of control group appeared significant trend on the high side, and the differences of systolic pressure, diastolic pressure and mean arterial pressure of the measurement of blood pressure between two groups were significant(t=3.047, t=3.126, t=2.975, P<0.01). Conclusion: The measurement of blood pressure for neonate often uses smaller oversleeve of blood pressure according subjective experience, while the choice of oversleeve of blood pressure as diameter meets objective standard. And it can assist medical care personnel to effectively obtain correct measurement value of blood pressure of neonate.
