摘　　要：目的:构建基于物联网技术的无纸化电子病案管理系统,实现诊疗综合信息数据的闭环管理。方法:采用浏览器与服务器(B/S)模式三层网络架构、WindowsServer2019服务器操作系统和甲骨文系统数据库,使用物联网射频识别(RFID)、全球定位系统及激光扫描器等信息传感设备,设置医生工作、护理工作、病历质量与安全管理及病历远程借阅等功能模块,设计基于物联网技术的无纸化电子病案管理系统,对管理系统上线两年来的数据进行分析,对比使用前后无纸化率、病历审查达标率和电子病历查询次数。结果:无纸化电子病案管理系统上线使用两年共接收电子病案150 250份,无纸化率达85.67%,各类病历用纸成本下降13%,病历形式审查达标率由原来的89%提高到100%。无纸化电子病案管理系统使用后病案无纸化率、病历形式审查达标率和电子病历查询次数较使用前明显提高,其差异有统计学意义(x^2=13.24,x^2=9.45,x^2=39.61;P<0.05)。结论:无纸化电子病案管理系统可实现物联网技术的医疗服务,解决患者就医信息数据的采集、统计和整合一体化管理问题,形成数据格式统一、数据互联共享的一站式电子病案管理体系,实现全程无缝记录患者就医信息数据。Objective: To construct a management system of paperless electronic medical record based on internet of things so as to realize closed-loop management of comprehensive information data of diagnosis and treatment. Methods: Adopting the three-layer network architecture of browser and Server(B/S) mode, Windows Server 2019 Server operating system and oracle system database, and using the internet of things radio frequency identification(RFID), Global Positioning System and laser scanners and other information sensing devices to set up a series of functional module included doctor work, nursing work, medical record quality and safety management, remote borrowing of medical record and so on, and to design paperless electronic medical record based on internet of things, and to analyze the data since two years of the system on line, and to compare the paperless rate, rate of reaching the standard of inspection for medical record and the query times of electrically medical record between pre and post using this system. Results: In the past two years of the paperless electronic medical record system on line, it had received 150 250 electronic medical records, and the paperless rate reached to 85.67%. The cost of each kind of medical record paper decreased 13%. The rate of reaching the standard of inspection for medical records was increased from 89% to 100%. After the paperless management system of electronic medical record was implemented, the paperless rate of medical record, the rate of reaching the standard of inspection for medical records and the query times of electrically medical record were significantly increased than those before this system was implemented(x^2=13.24, x^2=9.45, x^2=39.61, P<0.05).
