摘 要:碳青霉烯耐药菌在全球迅速蔓延的主要原因之一是耐药菌株产碳青霉烯酶。最流行的碳青霉烯酶是A类丝氨酸酶KPC(Klebsiella pneumoniae carbapenemase),其中以携带blaKPC-2的肺炎克雷伯菌最为流行。blaKPC-2编码基因通常存在于质粒上的单个拷贝中,有时也存在于染色体上,有最新的报道发现blaKPC-2也存在于单个质粒的三个拷贝中。国内外报道的blaKPC-2所处的基因环境有很大差异,表明其具有多态性。本文介绍了肺炎克雷伯菌携带的KPC-2基因环境的最新研究,并对多态性基因环境进行线性比对和分析。The carbapenem antibiotics are the last line of defense against Gram-negative bacteria.One of the main reasons for the rapid spread of carbapenem-resistant bacteria around the world is that the resistant strains produce carbapenemases.The most prevalent carbapenemases are Class A serine carbapenemases,which include Klebsiella pneumoniae carbapenemases(KPCs).Klebsiella pneumoniae with blaKPC-2 is the most prevalent,and it dramatically worsens clinical mortality and morbidity.However,the mechanism of its mediated gene transmission is still unknown,and this is a hot topic for scholars around the world.The blaKPC-2 gene microenvironment has been found to be the key to understanding its horizontal spread and controlling nosocomial infections,but the blaKPC-2 gene microenvironment is complex and diverse,and reports vary from country to region.Genes coding for the blaKPC-2 protein are usually found in a single copy of a plasmid and sometimes on a chromosome.A recent study reported that blaKPC-2 was also present in three copies of a single plasmid.The genetic microenvironment of KPC-2 as reported here in China and abroad differs widely,indicating polymorphisms.This article describes the latest research on the microenvironment of the KPC-2 gene carried by Klebsiella Pneumoniae,and linear alignment and analysis of the polymorphic gene microenvironment have been performed.
