摘 要:目的:建立利奈唑胺浓度高效液相色谱检测方法,研究其在颅脑外伤合并侧脑室外引流患者脑脊液中的药代动力学特征,为临床抗感染治疗提供参考依据。方法:采用Waters Spherisorb ODS2色谱柱(150 mm×4.6 mm,5μm),流动相:甲醇-水(40∶60),流速1.5 mL·min^-1,柱温25℃,紫外检测波长254 nm,进样量20μL。患者在口服利奈唑胺(600 mg,q 12 h)2 d后,检测给药后脑脊液样本中利奈唑胺的浓度。结果:利奈唑胺在0.05~40.00 mg·L^-1范围内线性关系良好(r=0.9995),用药2 d后2例患者脑脊液中利奈唑胺浓度达标,1例患者脑脊液中利奈唑胺浓度未达标需要调整用药方案。结论:利奈唑胺在颅脑外伤合并侧脑室外引流患者的脑脊液中有较宽泛的治疗范围,建议检测浓度,为临床抗感染治疗提供参考。Objective:To establish a method for the determination of linezolid concentration by high performance liquid chromatography(HPLC)method,and explore the pharmacokinetic characteristics of linezolid in cerebrospinal fluid(CSF)of patients with craniocerebral trauma and external drainage of lateral ventricle in order to provide references for clinical antiinfection treatment.Methods:The determination was performed on Waters Spherisorb ODS2 column(150 mm×4.6 mm,5μm)with mobile phase consisted of methanol-water(40∶60)at a flow rate of 1.5 mL·min^-1.The column temperature was 25℃,the detection wavelength was 254 nm,and the injection volume was 20μL.After orally taking linezolid(600 mg,q 12 h)for 2 days,the concentrations of linezolid in CSF of patients were detected.Results:Linezolid had a good linear relationship in the range of 0.05–40.00 mg·L^-1(r=0.9995).After two days of the treatment,the CSF concentration of linezolid in 2 patients reached the standard while that of 1 patient failed to reach the standard which was resulted to medication adjustment.Conclusion:Linezolid has a wide therapeutic range in CSF of patients with craniocerebral trauma and external drainage of lateral ventricle.It was recommended to detect the concentration of linezolid in CSF so as to provide references for clinical anti-infection treatment.
