摘  要:目的:了解并评价碳青霉烯类药物在某三级医院肿瘤/血液疾病患者中临床应用合理性,为合理用药提供参考。方法:采用回顾性研究方法,对2018年7月–2019年7月应用该药的361例肿瘤/血液疾病出院病例信息进行整理,收集患者基本信息及相关用药情况,结合相关文件评价用药合理性。结果:纳入病例中,符合用药适应证占96.12%;用法、用量及配伍不适宜占8.59%;未开展病原学送检占13.57%;未经特殊使用级抗菌药物专家会诊占5.26%。结论:该院碳青霉烯类抗菌药物在适应证、用法用量、品种选择及处方会诊方面较为合理,但应关注肾功能异常等特殊人群的合理用药,加强病原学送检。Objective:To evaluate the application of carbapenems in patients with tumor/hematologic diseases in a tertiary hospital,and provide references for rational drug use.Methods:The basic information and medication information of 361 patients with tumor/hematologic diseases discharged from a tertiary hospital from July 2018 to July 2019 were retrospectively collected,and the rationality of the use of carbapenems was evaluated based on the relevant documents.Results:Among the included cases,96.12%of the patients used carbapenems with reasonable indication.Rate of improper usage,dosage and compatibility of carbapenems were totally accounted for 8.59%.13.57%of the patients had no etiology test.5.26%of the patients were not consulted by experts for antibiotics of special use class.Conclusion:The application of carbapenems in this hospital was reasonable in indication,usage and dosage,variety selection and consultation.But we should pay attention to the rational use of drugs in special population(renal dysfunction)and strengthen the etiology test.
