延胡止痛方治疗带状疱疹后遗神经痛的疗效观察

摘　　要：【目的】观察延胡止痛方治疗肝经郁热证带状疱疹后遗神经痛(PHN)的临床疗效。【方法】将49例肝经郁热证PHN患者随机分为治疗组26例和对照组23例。治疗组给予中药延胡止痛方治疗,对照组给予口服普瑞巴林治疗,疗程为4周。观察2组患者治疗前后疼痛视觉模拟量表(VAS)评分和匹兹堡睡眠质量量表(PSQI)评分的变化情况,评价2组患者的临床疗效和安全性。【结果】(1)经1个疗程的治疗后,治疗组的总有效率为92.3%(24/26),对照组为78.3%(18/23),治疗组的总体疗效和总有效率均明显优于对照组,差异均有统计学意义(P<0.05)。(2)治疗后,2组患者的VAS评分和PSQI评分均较治疗前明显降低(P<0.05),且治疗组对VAS评分和PSQI评分的降低作用均明显优于对照组,差异均有统计学意义(P<0.05)。(3)安全性方面,治疗过程中,治疗组和对照组均未出现明显的不良反应,也均无脱落病例。【结论】口服中药延胡止痛方治疗肝经郁热证PHN疗效确切,可有效缓解临床症状,改善睡眠质量,其疗效优于使用普瑞巴林治疗的对照组。Objective To observe the clinical effect of Yanhu Zhitong Recipe for the treatment of postherpetic neuralgia. Methods Forty-nine postherpetic neuralgia patients with the syndrome of heat stagnated in liver meridian were randomly divided into treatment group(26 cases)and control group(23 cases). The treatment group was treated with modified Yanhu Zhitong Recipe and the control group was treated with Pregabalin tablets orally,4 weeks constituting a course of treatment. Before and after treatment,the scores of pain evaluated with visual analogue scale(VAS)and the scores of sleep quality evaluated with Pittsburgh sleep quality index(PSQI)in the two groups were examined. After treatment,the clinical efficacy and safety in the two groups were also evaluated.Results(1)After one course of the treatment,the total effective rate of the treatment group was 92.3%(24/26)and was significantly higher than that of the control group(78.3%,18/23),and the difference between the two groups was statistically significant(P<0.05).(2)After treatment,the VAS scores and PSQI scores of the two groups were decreased in comparison with those before treatment(P<0.05),and the treatment group had stronger effect on decreasing the VAS scores and PSQI scores than the control group,the difference between the two groups after treatment being statistically significant(P<0.05).(3)During the treatment,no obvious adverse reaction was found in the two groups. All of the patients completed the trial without falling off. Conclusion Oral use of Yanhu Zhitong Recipe exerts certain effect for the treatment of postherpetic neuralgia with the syndrome of heat stagnated in liver meridian,and it is more effective on relieving the clinical symptoms and improving the quality of sleep than Pregabalin tablets.
