雷火灸联合臭氧疗法治疗膝关节骨性关节炎疗效观察及对血清炎症因子的影响

摘　　要：【目的】观察雷火灸联合臭氧疗法治疗膝关节骨性关节炎的临床疗效,并探讨其对血清炎症因子的影响。【方法】将114例膝关节骨性关节炎脾肾两虚、湿注骨节证患者随机分为观察组和对照组,每组各57例。对照组给予臭氧疗法治疗,每周1次,共治疗5周;观察组在对照组治疗的基础上给予雷火灸治疗,每日1次,每周5次,共治疗5周。治疗5周后,评价2组临床疗效,观察2组患者治疗前后西安大略和麦克马斯特大学关节炎指数(WOMAC)评分、中医症状评分的变化情况,并检测2组患者治疗前后血清白细胞介素1(IL-1)、白细胞介素6(IL-6)以及肿瘤坏死因子α(TNF-α)的变化情况。【结果】(1)研究过程中,观察组脱落1例、剔除1例,对照组脱落1例、剔除2例。最终观察组55例,对照组54例纳入疗效统计。(2)观察组总有效率为92.73%(51/55),对照组为81.48%(44/54)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,2组患者的WOMAC指数评分、中医症状评分均明显改善(P<0.05),且观察组在改善WOMAC指数评分、中医症状评分方面均明显优于对照组,差异有统计学意义(P<0.05)。(4)治疗后,2组患者的血清IL-1、IL-6、TNF-α水平均明显改善(P<0.05),且观察组在改善血清IL-1、IL-6、TNF-α水平方面均明显优于对照组,差异有统计学意义(P<0.05)。【结论】雷火灸联合臭氧疗法可以有效缓解膝关节骨性关节炎患者的膝关节疼痛与肿胀、膝关节僵硬症状,改善患者的中医症状,提高患者的日常生活能力,并降低患者血清炎性因子水平,疗效显著。Objective To observe the clinical efficacy of moxibustion with immediate effect combined with ozone therapy for the treatment of knee osteoarthritis,and to explore its effects on serum inflammatory factors. Methods A total of 114 cases of knee osteoarthritis were randomly divided into observation group and control group,57 cases in each group. The control group received ozone therapy,once a week,for 5 weeks,and the observation group received moxibustion with immediate effect on the basis of the treatment of the control group,once a day,5 times a week,for 5 weeks. After 5 weeks of treatment,the clinical efficacy in the two groups was evaluated,and the changes of Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)and traditional Chinese medicine(TCM)symptom scores were observed in the two groups before and after treatment,and the serum levels of interleukin 1(IL-1),interleukin 6(IL-6)and tumor necrosis factor α(TNF-α)were detected in the two groups before and after treatment. Results(1)During the trial,one case dropped out and one case was get rid of from the observation group,one case dropped out and 2 cases were get rid of from the control group. Ultimately,55 cases in the observation group and 54 cases in the control group were included in the curative effect statistics.(2)The total effective rate was 92.73%(51/55)in the observation group and 81.48%(44/54)in the control group.The curative effect of the observation group was superior to that of the control group(P<0.05).(3)After treatment,WOMAC index scores and TCM symptom scores in the two groups were significantly improved(P<0.05), and the improvement in WOMAC index scores and TCM symptom scores of the observation group was significantly superior to that of the control group(P<0.05).(4)After treatment,the levels of serum IL-1,IL-6,TNF-α in the two groups was significantly improved(P<0.05),and the improvement in serum IL-1,IL-6,TNF-α levels of the observation group was significantly superior to that in the control group(P<0.05).Conclusion M
