摘　　要：总结李志山教授运用镇咳涤痰汤治疗小儿痉咳的临证经验。李志山教授认为小儿痉咳病位在肺、肝,与脾胃关系密切,表现为邪伏肺金,气津失布;肺病及脾,脾滞生痰;脾虚肝旺,肝旺侮金的病机演变过程。小儿患者具有发病容易,传变迅速的病理特点,一旦病邪进展,出现痉咳,则会导致肺、肝、脾胃同时发生变化,最终出现"木火刑金,风痰相搏"的核心病机。小儿痉咳的病理产物为痰、热、风,治疗以平肝泻肺、涤痰降逆为法;李志山教授创制镇咳涤痰汤(主要由青礞石、黛蛤散、桑白皮、制半夏、炙百部、枇杷叶、生甘草等组成),随证加减治疗小儿痉咳。The experience of Professor LI Zhi-Shan in treating infantile spasmodic cough with Zhenke Ditan Decoction, a herbal compound prescription with the actions of relieving cough and removing phlegm, was summarized. Professor LI Zhi-Shan believes that the infantile spasmodic cough is involved in the lung and liver,and the disease is closely related with the spleen and stomach. The pathogenesis evolution process is manifested as pathogenic factors lingering in the lung resulting into the disordered distribution of qi and fluid, lung disease involving the spleen and then leading to the formation of phlegm due to the spleen qi stagnation,deficiency spleen leading to hyperactive liver, and then the hyperactive liver counter-restricting the spleen. The children are susceptible to being attacked by the pathogens and infantile diseases have the tendency of developing rapidly. Once the pathogenic attack develops into the spasmodic cough, simultaneous injury of the lung, liver, spleen and stomach will occur,which will cause the formation of the key pathogenesis of"liver-fire impairing the lung,and struggle between liver wind and phlegm". The pathological products of infantile spasmodic cough are phlegm,heat and liver wind. Based on the principle therapy of calming liver,purging lung,removing phlegm,and lowering adverse qi, Professor LI Zhi-Shan has established the effective recipe of Zhenke Ditan Decoction by modified utilization on the basis of syndrome differentiation. And the recipe is mainly composed of Lapis Chloriti,Daige Powder,Cortex Mori,Rhizoma Pinelliae Preparata,Radix Stemonae Preparata,Folium Eriobotryae,and Radix Glycirrhizae.
