加速康复理念在手汗症单孔胸腔镜手术中的应用

摘  要:目的探讨加速康复理念(enhanced recovery after surgery,ERAS)应用在单孔胸腔镜(video-assisted thoracic surgery,VATS)手汗症手术中的可行性及临床价值。方法回顾性分析我院自2017年6月—2019年10月单孔胸腔镜手术治疗的52例手汗症患者资料,患者在围手术期均采用了加速康复外科的一系列措施。结果52例患者均未发生术中大出血等严重并发症而中转开胸;术后无迟发型气胸、胸腔积液;出院后没有因手术相关并发症二次入院患者;术后无恶心、呕吐及咽喉部不适;手术时间25~86 min,平均(52.3±18.6)min;手术出血量10~30 mL,平均(12.2±6.5)mL;术后疼痛评分(2.3±0.4)分。结论加速康复理念应用于单孔胸腔镜手汗症手术是安全可行的,可有效加快患者恢复和缩短住院时长,改善患者术后舒适度。Objective To explore the feasibility and clinical value of the application of the concept of accelerated rehabilitation in the operation of single hole thoracoscopic palmar hyperhidrosis.Methods The data of 52 cases of palmar hyperhidrosis treated by uniportal-VATS from June 2017 to October 2019 in our hospital were analyzed retrospectively.The patients adopted a series of measures of accelerating rehabilitation surgery in the perioperative period.Results No serious complications such as intraoperative massive hemorrhage occurred in 52 patients,no delayed pneumothorax and pleural effusion occurred after operation,no patients were admitted to hospital for operation related complications after discharge,no nausea,vomiting and throat discomfort were found after operation;The mean operation time was(52.3±18.6)min(range,25~86 min);The average blood loss was(12.2±6.5)mL(range,10~30 mL);The postoperative pain score was(2.3±0.4).Conclusion It is safe and feasible to apply the concept of accelerated rehabilitation in single port thoracoscopic surgery for palmar hyperhidrosis,which can effectively accelerate the recovery of patients,shorten the length of hospital stay,and improve the postoperative comfort of patients.
