复发性肩关节前方不稳定MRA表现与肩关节镜的对比研究

摘　　要：目的探索MRA对复发性肩关节前方不稳定诊断的敏感度、特异性及准确度。方法2018年4月—2019年4月通过对16例复发性肩关节前方不稳定患者病例资料进行回顾性分析,将MRA检测结果与肩关节镜结果进行对比,以肩关节镜结果为“金标准”,分析肩关节前盂唇撕裂、肩关节前方盂缘损伤、肩关节盂肱韧带异常、肩关节Hill-sachs损伤MRA结果对肩关节镜的敏感度、特异性及准确度。结果肩关节前盂唇撕裂、肩关节前方盂缘损伤、肩关节盂肱韧带异常、肩关节Hill-sachs损伤MRA结果对肩关节镜的敏感度分别为90.0%、81.8%、87.5%、100.0%;特异性分别为80.0%、80.0%、75.0%、100.0%;准确度分别为90.9%、90.0%、77.8%、100.0%。结论MRA对复发性肩关节前方不稳定的诊断有较高的敏感度、特异性及准确度,对于临床诊断具有重要意义。Objective To explore the sensitivity,specificity and accuracy of MRA for the diagnosis of recurrent anterior shoulder instability.Methods By retrospectively analyzing the data of 16 patients from April 2018 to April 2019 with recurrent anterior shoulder joint instability,the MRA test results were compared with the results of shoulder arthroscopy,and the results of shoulder arthroscope as the"gold standard"were used to analyze the anterior glenoid tear of the shoulder joint.The sensitivity,specificity and accuracy of MRA results for shoulder arthroscope,anterior glenoid injury of the shoulder joint,abnormality of the shoulder glenoid brachial ligament,and shoulder-sachs injury.Results The anterior glenoid lip of the shoulder joint,anterior glenoid injury of the shoulder joint,anomaly of the glenohumeral ligament of the shoulder joint,and Hill-sachs injury of the shoulder joint.The sensitivity of the MRA results to the shoulder arthroscopy were 90.0%,81.8%,87.5%,and 100.0%;Specificity were 80.0%,80.0%,75.0%,100.0%;accuracy were 90.9%,90.0%,77.8%,100.0%.Conclusion MRA has high sensitivity,specificity and accuracy for the diagnosis of recurrent anterior shoulder joint instability,which is of great significance for clinical diagnosis.
