PFNA微创治疗老年股骨粗隆间骨折的疗效研究

摘　　要：目的分析PFNA微创治疗老年股骨粗隆间骨折的临床治疗效果。方法方便收集2016年7月—2019年6月间在该院进行治疗的老年股骨粗隆间骨折患者70例,按照随机数字表法将其分为对照组和观察组,其中将实施微创DHS内固定治疗的组别设为对照组,将实施PFNA内固定治疗的组别设为观察组,记录两组患者手术时间、术中出血量、住院时间、负重时间和愈合时间,并观察两组患者术后并发症发生情况,对比临床治疗效果。结果与对照组相比,观察组患者手术时间(67.82±3.06)min、术中出血量(180.77±8.20)mL、住院时间(15.22±1.02)d、负重时间(5.33±0.51)周以及愈合时间(10.23±1.21)周均明显更佳(t=22.204、54.398、16.166、16.481、13.975,P<0.05);与对照组(74.29%)相比,观察组患者治疗总有效率(94.29%)明显更高,差异有统计学意义(χ^2=5.285,P<0.05)。结论针对老年股骨粗隆间骨折患者,临床给予PFNA内固定治疗效果明显优于微创DHS内固定,有助于缩短手术时间和住院时间,减少术中出血量,加快患者负重和愈合速度,值得临床大力推广应用。Objective To analyze the clinical effect of PFNA minimally invasive treatment of intertrochanteric fractures in the elderly.Methods Convenient selection of 70 elderly patients with intertrochanteric fractures treated in our hospital from July 2016 to June 2019,and divide them into a control group and an observation group according to the random number table method,in which minimally invasive DHS implemented with internal fixation treatment was set as the control group,and the group with PFNA internal fixation treatment was set as the observation group.The operation time,intraoperative blood loss,hospitalization time,weight bearing time and healing time of the two groups of patients were recorded,and the two groups were observed.The postoperative complications of patients were compared with the clinical treatment effect.Results Compared with the control group,the operation time of the observation group(67.82±3.06)min,intraoperative blood loss(180.77±8.20)ml,hospital stay(15.22±1.02)d,weight-bearing time(5.33±0.51)wwek and healing time(10.23±1.21)wwek was significantly better(t=22.204,54.398,16.166,16.481,13.975,P<0.05);compared with the control group(74.29%),the total effective rate of treatment in the observation group(94.29%)was significantly better High,the difference was statistically significant(χ^2=5.285,P<0.05).Conclusion For elderly patients with femoral intertrochanteric fractures,the clinical effect of PFNA internal fixation is significantly better than that of minimally invasive DHS internal fixation.It is worthy of clinical promotion and application.
