摘  要:目的探讨持续呼吸道正压通气(continuous positive airway pressure,CPAP)联合枸橼酸咖啡因治疗新生儿呼吸窘迫综合征的临床疗效。方法方便选取该院(2018年1月—2020年1月时期)收治55例新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)患儿,采取随机数字法分成两组,其中以CPAP联合枸橼酸咖啡因治疗设置为咖啡组(28例),以CPAP联合氨茶碱治疗设置为氨茶碱组(27例)。对照两组患儿治疗前、后血氧饱和度水平,CPAP使用时间,住院时间,临床治疗效果,并发症发生情况、喂养不耐受及撤机失败率比较。结果经治疗后两组患儿血氧饱和度水平均较治疗前提升,其中咖啡组患儿血氧饱和度水平明显高于氨茶碱组患儿,且咖啡组患儿CPAP使用时间、住院时间明显短于氨茶碱组患儿(P<0.05)。咖啡组患儿临床治疗总有效率为96.43%,高于氨茶碱组患儿临床治疗总有效率74.07%(χ^2=5.526,P<0.05)。咖啡组患儿并发症发生率低于氨茶碱组患儿并发症发生率(P<0.05)。咖啡组患儿喂养不耐受率、撤机失败率与氨茶碱组相比较,差异无统计学意义(P>0.05)。结论在NRDS的治疗中,以CPAP联合枸橼酸咖啡因治疗,可明显的提高患儿的血氧饱和度水平,缩短患儿的CPAP使用时间及住院时间,提升患儿的临床治疗效果,明显降低并发症发生率效果理想。Objective To explcre the clinical efficacy of(Continueous positive airway pressure,CPAP)combined with caffeine citrate in the treatment of neonatal respiratory distress syndrome.Methods From January 2018 to January 202055 patients with neonatal respiratory distress syndrome(NRDS)were treated in the hospital.They were divided into two groups by random number method.The acid caffeine treatment was set in the coffee group(28 cases),and the CPAP combined with aminophylline treatment was set in the aminophylline group(27 cases).Before and after treatment,the blood oxygen saturation level,CPAP use time,hospital stay,clinical treatment effect,complications,feeding intolerance and withdrawal failure rate were compared between the two groups.Results After treatment,the blood oxygen saturation level of the two groups of patients was higher than before treatment.The blood oxygen saturation level of the coffee group was significantly higher than that of the aminophylline group,and the CPAP use time and hospitalization time of the coffee group children,significantly shorter than children in the aminophylline group(P<0.05).The total effective rate of clinical treatment in children in the coffee group was 96.43%,which was higher than the total effective rate of clinical treatment in children in the aminophylline group 74.07%(χ^2=5.526,P<0.05).The incidence of complications in the coffee group was lower than that in the aminophylline group(P<0.05).Compared with the aminophylline group,the feeding intolerance rate and withdrawal failure rate of the children in the coffee group were not statistically significant(P>0.05).Conclusion In the treatment of NRDS,CPAP combined with caffeine citrate treatment can significantly increase the blood oxygen saturation level of the child,shorten the CPAP use time and hospital stay of the child,and improve the clinical treatment effect of the child.The effect of reducing the incidence of complications is ideal.
