摘　　要：目的探讨迈瑞CAL 8000血液分析流水线(由BC-6800、BC-69002台血细胞分析仪和1台SC-1200全自动制片染色机组成)联合血涂片镜检在血常规检验中的应用价值。方法收集2017年3月汶上县人民医院门诊和住院患者及体检人员的3775份血液标本,经血液分析流水线筛选复检标本,参考国际血液学复检专家组制定的自动血细胞计数和白细胞分类计数复检规则,根据该院实际情况制定临床实验室血涂片复检规则;另选同期本院患者及体检人员的1080份血液样本,用于评价检验科血涂片复检规则的可行性和可靠性。结果制定的血涂片复检规则涉及白细胞计数(WBC)、血红蛋白(Hb)、血小板计数(PLT)、单核细胞比例等指标异常,复检方法使用涂片镜检或人工分类镜检。参照复检标准,1080份标本经血液分析流水线筛选出112份需要血涂片染色镜检复查的标本,复检率为10.4%,假阴性率为3.9%(42/1080)。使用该复检规则,2017年4—9月检出389例阳性标本,其中血液病14例,未发现白血病漏检。结论制定符合各医院实际情况的血细胞分析仪复检标准有重要的临床意义,能有效避免血液系统疾病的漏检,提高工作效率,为临床诊治提供有价值的信息。Objective To investigate the application value of Mairui CAL 8000 blood analysis assembly line(composed of BC-6800 and BC-6900 blood cell analyzers and SC-1200 automatic film-making and dyeing machine)combined with blood smear microscopy in blood routine examination.Methods The 3775 blood samples were collected from outpatients,inpatients and physical examiners in People's Hospital of Wenshang County in March 2017,and the samples were screened by blood analysis line.Referring to the automatic blood count and white blood cell classification reexamination rules developed by the International Hematology Reexamination Expert Group,the blood smear reexamination rules were formulated according to the actual situation of the hospital.Other 1080 blood samples from patients and physical examiners of the hospital during the same period were selected to evaluate the feasibility and reliability of the blood smear reexamination plan made by clinical laboratory.Results The blood smear reexamination rules included white blood cell count(WBC),hemoglobin(Hb),platelet count(PLT),monocyte percentage and other indicators,and the reexamination method was smear microscopy or manual classification microscopy.According to the standard of reexamination,112 samples were screened out from 1080 samples by blood analysis line.The reexamination rate was 10.4%,and the false negative rate was 3.9%(42/1080).According to the reexamination rules,389 positive samples were detected from April to September in 2017,including 14 cases of hematologic diseases,and no leukemia was missed.Conclusion It is of great clinical significance to formulate the reexamination standard of blood cell analyzer in accordance with the actual situation of each hospital,which can effectively avoid the missed diagnosis of blood system diseases,improve work efficiency and provide valuable information for clinical diagnosis and treatment.
