摘　　要：目的探讨微RNA-128(microRNA-128,miRNA-128)表达与冠心病患者冠状动脉粥样斑块稳定性的相关性。方法选取2017年6月至2019年6月连云港市第一人民医院收治的冠心病患者132例为研究对象,根据血管内超声射频数据分析结果将入选患者分为稳定斑块组(35例)和易损斑块组(97例),检测两组患者血清miRNA-128表达量,采用受试者操作特征曲线(receiver operating characteristic curve,ROC曲线)分析miRNA-128表达量对易损斑块的预测价值。结果易损斑块组患者miRNA-128表达量显著高于稳定斑块组(P=0.04),斑块偏心指数、斑块负荷、坏死核心(necrotic core,NC)比率、钙化部分(dense calcium,DC)比率、纤维脂质成分(fibro-fatty,FF)比率均显著大于稳定斑块组(均P<0.05)。Pearson相关性分析显示:血清miRNA-128表达量与斑块偏心指数、斑块负荷、NC比率、DC比率、FF比率均呈显著正相关(r分别为0.563、0.632、0.701、0.724、0.612,均P<0.05)。ROC曲线分析显示:miRNA-128高表达对冠心病患者出现易损斑块具有较好的预测价值,曲线下面积为0.79,灵敏度为67%,特异度为82%。结论miRNA-128表达与冠心病患者冠状动脉粥样斑块稳定性具有相关性,miRNA-128高表达对出现易损斑块具有较高的预测价值。Objective To explore the correlation between microRNA-128(miRNA-128)and the stability of coronary atheromatous plaque in patients with coronary artery heart disease.Method From June 2017 to June 2019,132 patients with coronary artery heart disease were divided into stable plaque group(n=35)and vulnerable plaque group(n=97)according to the results of intravascular ultrasound and radio-frequency data analysis.The expression of serum miRNA-128 was measured and the predictive value of miRNA-128 expression for vulnerable plaque was analyzed by receiver operating characteristic curve(ROC curve).Result The expression of miRNA-128 in vulnerable plaque group was significantly higher than that in stable plaque group(P=0.04).The plaque eccentricity index,plaque load,necrotic core(NC)ratio,dense calcium(DC)ratio and fibro-fatty(FF)ratio in vulnerable plaque group were significantly higher than those in stable plaque group(all P<0.05).Pearson correlation analysis showed that serum miRNA-128 expression was positively correlated with plaque eccentricity index,plaque load,NC ratio,DC ratio and FF ratio(r=0.563,0.632,0.701,0.724,0.612,all P<0.05).ROC curve analysis showed that the high expression of miRNA-128 had high predi ctive value for the occurrence of vulnerable plaques of coronary artery heart disease patient:the area under the curve was 0.79,the sensitivity was 67%,and the specificity was 82%.Conclusion There is a correlation between the expression of miRNA-128 and the stability of coronary atheromatous plaque in patients with coronary artery heart disease,and the high expression of miRNA-128 has a high predictive value for the occurrence of vulnerable plaque.
