腹腔镜手术治疗急性阑尾炎合并糖尿病的临床效果研究

摘　　要：目的观察腹腔镜手术治疗急性阑尾炎合并糖尿病的临床效果。方法选取2017年1月-2019年9月该院普外科诊治72例急性阑尾炎合并糖尿病患者,随机分为对照组36例(传统开腹手术)和观察组36例(腹腔镜手术),对比两组治疗效果、血糖水平及并发症发生的情况。结果观察组手术时间、术中出血量、胃肠功能恢复时间、首次排气时间、术后住院时间等各项指标全面优于对照组的相应指标,差异有统计学意义(P<0.05);观察组术前2 h血糖水平为(9.61±2.38)mmol/L与对照组的(9.58±2.16)mmol/L基本相同,差异无统计学意义(P>0.05);观察组术后随机血糖(6.62±1.86)mmol/L,明显优于对照组(8.82±2.27)mmol/L,差异有统计学意义(P<0.05);观察组并发症发生率为13.89%,明显低于对照组的41.67%,差异有统计学意义(P<0.05)。结论对急性阑尾炎合并糖尿病患者实施腹腔镜手术,相比传统开腹手术疗效更好,安全性更高,又能降低术后并发症发生率,利于患者尽快地康复。Objective To observe the clinical effect of laparoscopic surgery on acute appendicitis complicated with diabetes.Methods From January 2017 to September 2019,72 patients with acute appendicitis and diabetes mellitus diagnosed and treated in our hospital were randomly divided into the control group(traditional open surgery)and the observation group 36(laparoscopic surgery).Treatment effects,blood glucose levels,and complications occurred in the two groups.Results The operation group,blood loss during operation,gastrointestinal function recovery time,first exhaust time,and postoperative hospital stay were significantly better than those in the control group,with significant differences(P<0.05);2 h preoperative blood glucose level in the observation group was(9.61±2.38)mmol/L and(9.58±2.16)mmol/L in the control group was basically the same,there was no significant difference(P>0.05);random blood glucose in the observation group after operation(6.62±1.86)mmol/L,significantly better than the control group(8.82±2.27)mmol/L,the differences were statistically significant(P<0.05);the incidence of complications in the observation group was 13.89%,which was significantly lower than that in the control group 41.67%,the difference was statistically significant(P<0.05).Conclusion Laparoscopic surgery for patients with acute appendicitis combined with diabetes is more effective and safer than traditional open surgery.It can also reduce the incidence of postoperative complications and help patients recover as soon as possible.
