经皮椎弓根钉外固定系统联合椎体成形术治疗胸腰椎骨折的效果研究

摘　　要：目的探讨经皮椎弓根钉外固定系统联合椎体成形术治疗胸腰椎骨折的临床效果。方法70例胸腰椎骨折患者作为研究对象,随机分为对照组和治疗组,各35例。对照组行传统椎弓根钉内固定术治疗,治疗组行经皮椎弓根钉外固定系统联合椎体成形术治疗。对比两组并发症发生情况及治疗前后椎体高度、Cobb’s角。结果治疗前,两组患者椎体高度与Cobb’s角对比,差异无统计学意义(P>0.05);治疗2年后,两组患者的椎体高度与Cobb’s角均优于治疗前,且治疗组椎体高度(20.93±2.53)mm、Cobb’s角(6.85±3.22)°优于对照组的(18.73±2.93)mm、(9.45±2.93)°,差异有统计学意义(P<0.05)。治疗组患者术后并发症发生率为5.71%,低于对照组的22.86%,差异有统计学意义(P<0.05)。结论经皮椎弓根钉外固定系统联合椎体成形术治疗胸腰椎骨折效果显著,远期疗效好,并发症少,值得推广。Objective To discuss the clinical effect of percutaneous pedicle screw external fixation system combined with vertebroplasty on thoracolumbar fractures.Methods A total of 70 cases of thoracolumbar fractures patients as study subjects were randomly divided into control group and treatment group,with 35 cases in each group.The control group was treated with traditional pedicle screw internal fixation,and the treatment group was treated with percutaneous pedicle screw external fixation system combined with vertebroplasty.The occurrence of complications,and vertebral height and Cobb’s angle before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in vertebral height and Cobb’s angle between the two groups(P>0.05).2 years after treatment,vertebral height and Cobb’s angle of the two groups was better than that before treatment,and the treatment group[(20.93±2.53)mm,(6.85±3.22)°]was better than the control group[(18.73±2.93)mm,(9.45±2.93)°],and the difference was statistically significant(P<0.05).The incidence of postoperative complications of the treatment group was 5.71%,which was lower than that of the control group 22.86%,and the difference was statistically significant(P<0.05).Conclusion Percutaneous pedicle screw external fixation system combined with vertebroplasty has a significant effect in the treatment of thoracolumbar fractures,with good long-term curative effects and few complications,which is worthy of promotion.
