摘 要:目的分析探讨颅脑CT对急性脑外伤患者手术疗效及预后的评估价值。方法研究对象便利选取自2016年11月-2019年11月该院所接收的急性脑外伤患者65例,对这65例患者使用手术治疗,并通过颅脑CT检查,对65例患者的颅脑CT主要表现进行分析、分析65例患者的CT分型结果、分析65例患者的预后结果与患者CT分型的关系。结果在65例急性脑外伤患者中,闭合性损伤61例,开放性损伤4例,其中出现硬膜外出血21例,硬膜下出血19例,硬膜外血肿并伴有硬膜下血肿12例,硬膜下血肿并伴有脑挫裂伤8例,脑内血肿并伴有脑挫裂伤6例,出现脑室内出血2例;CT分型为Ⅰ型的患者27例(41.54%),Ⅱ型20例(30.77%),Ⅲ型9例(13.85%),Ⅳ型6例(9.23%);预后较为良好的患者中,CT分型为Ⅰ型、Ⅱ型的患者为37(92.50%),明显高于预后较差,CT分型为Ⅰ型、Ⅱ型的患者13(52.00%),差异有统计学意义(χ^2=4.840,P=0.030<0.05)。结论通过颅脑CT的使用,可对患者进行减压手术前后的颅内病变改善情况分型,对患者的手术疗效进行预测,并分析其预后,为患者提前做好准备措施,具有较高的临床应用价值,值得推广使用。Objective To analyze and discuss the value of craniocerebral CT in the evaluation of surgical effect and prognosis of patients with acute brain injury. Methods 65 patients with acute brain injury received by the hospital from November 2016 to November 2019 were convenient selected for the study. The main manifestations of craniocerebral CT in65 patients were analyzed and the CT types of 65 patients were analyzed through the examination of craniocerebral CT.Results Of the 65 patients with acute head injury, 61 had closed injury and 4 had open injury, including 21 with epidural hemorrhage, 19 with subdural hemorrhage, and 12 with epidural Hematoma and head injury, there were 8 cases of head injury with cerebral contusion and laceration, 6 cases of intracerebral Hematoma with cerebral contusion and laceration, 2 cases of intraventricular hemorrhage, 27 cases(41.54%) with CT type Ⅰ, 20 cases(30.77%) with type Ⅱ, 9 cases(13.85%)with type Ⅲ and 6 cases(9.23%) with type Ⅳ among the patients with good prognosis, 37(92.50%) were classified as type Ⅰ and Type Ⅱ by CT, which was significantly higher than the poor prognosis. 13(52.00%) were classified as type Ⅰ and Type Ⅱ by CT, the difference was statistically significant(χ~2=4.840, P=0.030 <0.05). Conclusion Through the use of brain CT, the improvement of intracranial lesions before and after decompression operation can be classified, the surgical effect of patients can be predicted, and the prognosis can be analyzed, so as to prepare measures for patients in advance, which has high clinical application value and is worth popularizing.
