微创小切口切开复位钢板内固定术在肱骨骨折治疗中的应用效果分析

摘　　要：目的评价微创小切口切开复位钢板内固定术在肱骨骨折治疗中的应用效果。方法对该院2018年4月-2019年4月收治的肱骨骨折患者进行筛选,从中方便选择64例肱骨骨折患者作为该次研究的对象,系统随机对患者进行编号,奇数为研究组,偶数为对照组。对照组患者采用传统切开复位内固定术治疗,研究组患者采用微创小切口切开复位钢板内固定术治疗。术后随访半年,比较两组术中出血量、住院时间、住院费用、术后VAS评分、并发症发生率、NERR评分优良率。结果经治疗,研究组术中出血量(87.23±10.28)mL、住院时间(10.20±0.41)d、住院费用(2.1±0.62)万元、VAS评分(2.08±0.22)分优于对照组(206.84±10.65)mL、(15.60±0.39)d、(2.9±0.58)万元、(4.73±0.25)分,差异有统计学意义(t=4.013、3.828、3.702、3.769,P<0.05);研究组术后并发症发生率3.13%低于对照组18.75%,NEER评分优良率93.75%高于对照组78.13%,差异有统计学意义(χ^2=5.026、6.203,P<0.05)。结论对肱骨骨折患者实施微创小切口切开复位钢板内固定术能够减少术中出血量、减轻术后疼痛与经济压力,可以缩短患者住院时间,降低并发症发生率,与传统术式相比关节恢复情况更好,值得应用。Objective To evaluate the effect of minimally invasive small incision open reduction and plate internal fixation in the treatment of humeral fracture.Methods From April 2018 to April 2019,64 patients with humeral fracture were convenient selected as the subjects of this study.The patients were randomly numbered.The odd number was the study group and the even number was the control group.Patients in the control group were treated with traditional open reduction and internal fixation,while patients in the study group were treated with minimally invasive small incision open reduction and internal fixation with steel plate.After six months follow-up,the bleeding volume,length of stay,cost of hospitalization,VAS score,complication rate and nerr score were compared between the two groups.Results After treatment,the bleeding volume(87.23±10.28)mL,hospitalization time(10.20±0.41)d,hospitalization cost(2.1±0.62)ten thousand yuanand VAS score(2.08±0.22)points in the study group were better than those in the control group(206.84±10.65)mL,(15.60±0.39)d,(2.9±0.58)ten thousand yuanand,(4.73±0.25)points,with statistical differences(t=4.013,3.828,3.702,3.769,P<0.05);the incidence of postoperative complications in the study group was 3.13%lower than that in the control group 18.75%,the excellent and good rate of neer score was 93.75%higher than that of the control grou 78.13%(χ^2=5.026,6.203,P<0.05).Conclusion Minimally invasive small incision open reduction and plate internal fixation for humerus fracture patients can reduce the amount of intraoperative bleeding,reduce postoperative pain and economic pressure,shorten the length of stay of patients and reduce the incidence of complications.Compared with the traditional operation,the joint recovery is better,which is worthy of application.
