临床筛查与Graf超声检查在发育性髋关节发育不良儿童保健门诊中的应用

摘　　要：目的:探讨临床筛查在发育性髋关节发育不良(DDH)儿童保健门诊中的作用.方法:对在我院就诊的年龄在6个月以内进行的体检儿童进行DDH临床筛查,并对临床筛查异常的患儿按照体格检查和高危因素的组合排列分为A组(体格检查正常+至少1个高危因素组)、B组(体格检查异常+至少1个高危因素组)、C组(体格检查异常+无高危因素组)3组,每组各100例,对不同组别的患儿进一步完善Graf超声检查,比较各组间DDH检出率的统计学差异.结果:A组患儿有10例确诊为DDH,B组患儿有12例确诊为DDH,C组患儿中2例确诊为DDH,3组患儿DDH检出率之间存在统计学差异(P<0.05),其中A组与C组之间DDH检出率有统计学差异(P=0.016),B组与C组之间发病率有统计学差异(P=0.006),A组与B组发病率之间无统计学差异(P=0.651).结论:儿童保健门诊对发育性髋关节发育不良的患儿筛查中,无论体格检查有无阳性体征,需要进一步询问有无发病的高危因素,临床筛查异常的患儿均建议完善Graf超声检查,避免误诊漏诊.Objective:To explore the role of clinical screening in children's health clinic of developmental dysplasia of the hip(DDH).Method:DDH clinical screening was carried out for physical examination children who were in our hospital within 6 months.According to the combination of physical examination and high-risk factors,the children with abnormal clinical screening were divided into three groups:group A(normal physical examination+at least one high-risk factor group),group B(abnormal physical examination+at least one high-risk factor group)and group C(abnormal physical examination+no high-risk factor group),Each group of 100 cases,different groups of children to further improve the Graf ultrasound examination,and compare the statistical differences of DDH detection rate between groups.Results:Group A,10 cases were diagnosed as DDH,12 cases of group B were diagnosed as DDH,2 cases of group C were diagnosed as DDH,there was a significant difference between the three groups(P<0.05),there was a significant difference be-tween the A group and C group(DDH),there was a significant difference in incidence rate between group A and group B(P),and there was no significant difference between incidence rate of group C and group A.Conclusion:In the screening of children with developmental dysplasia of the hip in the children's health care clinic,whether there is positive physical signs or not,it is necessary to further inquire whether there are high-risk factors of the disease.For children with abnormal clinical screening,it is suggested to improve Graf ultrason-ic examination to avoid misdiagnosis and missed diagnosis.
