摘  要:目的:探讨多层螺旋CT三维重建技术在骨关节骨折检出率中的应用价值。方法:2017年10月至2018年8月168例骨关节骨折患者均首先行常规DR检查,对于DR未能明确诊断或未发现的骨折,行多层螺旋CT轴位辅以多平面重建(MPR)、容积重建(VR)、曲面重建(CPR)、表面遮盖成像(SSD)等后处理技术进行诊断。结果:常规DR准确检出胫骨平台骨折36例,桡骨远端骨折29例,髋关节骨折25例,踝关节骨折14例,骨盆骨折9例,肩关节骨折13例,肋骨骨折5例,膝关节骨折4例,下颌骨折3例,脊柱骨折2例,共准确检出140例。结论:多层螺旋CT三维重建技术对于解剖关系复杂、骨折部位隐匿的骨折具有重要的诊断价值,是常规X线及CT扫描的重要补充。Objective:To investigate the application value of three-dimensional reconstruction of multi-slice spiral CT in the detection rate of bone and joint fractures.Methods:From October 2017 to August 2018,168 patients with bone and joint fractures underwent routine DR examinations.For fractures that were not clearly diagnosed or undetected by DR,multi-slice spiral CT was performed axially supplemented with multiplanar reconstruction(MPR),volume reconstruction(VR),surface reconstruction(CPR),surface mask imaging(SSD)and other post-processing techniques for diagnosis.Results:Routine DR accurately detected 36 cases of tibial plateau fractures,29 cases of distal radius fractures,25 cases of hip fractures,14 cases of ankle fractures,9 cases of pelvic fractures,13 cases of shoulder fractures,5 cases of rib fractures,and knee joints.There were 4 cases of fractures,3 cases of mandibular fractures,and 2 cases of spine fractures.A total of 140 cases were accurately detected.Conclusion:Multi-slice spiral CT three-dimensional reconstruction technology has important diagnostic value for fractures with complex anatomical relationships and hidden fracture sites,and is an important supplement to conventional X-ray and CT scans.
