摘  要:目的:分析基于低频振幅(ALFF)的静息态功能MR成像在无灶性癫痫(NLE)患者大脑功能评价中的应用价值。方法:抽取45例NLE患者、40例健康志愿者分别为NLE组、对照组,均于常规MRI扫描后获取轴位静息态fMRI功能像,ALFF分析获取每个体素在经典频段(0.010~0.080Hz)、slow-4频段(0.027~0.073Hz)及slow-5频段(0.010~0.027Hz)的标准化ALFF值,比较两组大脑自发活动差异。结果:与对照组相比,NLE组经典频段ALFF值增高的脑区为左侧颞中回,减低的脑区主要包括左侧前扣带回、眶部额下回及右侧小脑后叶、岛叶和顶上回。NLE组slow-4频段ALFF值增高的脑区为左侧颞中回,slow-5频段ALFF值增高的脑区为双侧颞中回;slow-4频段ALFF值减低的脑区包括左侧前扣带回、眶部额下回与右侧小脑后叶、岛叶、额中回,slow-5频段ALFF值减低的脑区包括左侧前扣带回、颞极(颞上回)、背外侧额上回与右侧楔前叶、顶下小叶。结论:基于ALFF的静息态功能MR成像可反映NLE患者大脑内部神经元自发活动的广泛异常改变,部分脑区自发活动与频率改变有关,各频段可互补地发现异常活动脑区。Objective:To analyze the application value of resting-state functional MR imaging based on amplitude of low-frequency fluctuation(ALFF)on the evaluation of brain function in patients with non-lesional epilepsy(NLE).Methods:Forty-five NLE patients and 40 healthy volunteers were selected as NLE group and control group.Axis resting fMRI functional images were obtained after routine MRI scanning.ALFF analysis was used to obtain standardized ALFF values of each voxel in classical frequency band 0.01~0.08 Hz,slow-4 frequency band 0.027~0.073 Hz and slow-5 frequency band 0.01~0.027 Hz,which were larger than those of the two groups.Differences in spontaneous brain activity.Results:Compared with the control group,the brain area with the increased ALFF value in the classic band of the NLE group was the left middle temporal gyrus,the decreased brain area mainly included the left anterior cingulate gyrus,orbital frontal gyrus and the right posterior cerebellar lobe,insula lobe and top gyrus.At the same time,the brain area with the increased ALFF value in the low-4 band of the NLE group was the left middle temporal gyrus,the brain area with the increased ALFF value in the slow-5 band was the bilateral middle temporal gyrus.the brain areas with the reduced ALFF value in the slow-4 band included the left anterior cingulate gyrus,orbital frontal gyrus and the right posterior cerebellar lobe,insula lobe and middle frontal gyrus.The brain areas with reduced ALFF values in the slow-5 band included the left anterior cingulate gyrus,temporal pole(superior temporal gyrus),dorsolateral superior frontal gyrus,the right precuneus and inferior parietal lobule.Conclusion:Resting-state functional MR imaging based on ALFF can reflect the extensive abnormal changes of spontaneous activity of neurons in the brain of NLE patients.
