摘　　要：目的探讨MOSES大功率钬激光前列腺剜除术(HoLEP)治疗良性前列腺增生(BPH)的安全性与疗效性。方法选取2018年10月至2019年2月本院收治的120例BPH患者,随机分为MOSES大功率钬激光前列腺剜除术治疗组(MOSES-HoLEP组)和前列腺等离子电切术治疗组(PKRP组),每组各60例。收集临床资料,并分析比较两组的手术时间、切除标本重量、血红蛋白下降幅度、膀胱冲洗时间、留置尿管时间、住院时间、残余尿量(PRV)、最大尿流率(Q)、国际前列腺症状评分(IPSS)、生活质量评分(Q0L)以及并发症的发生情况。结果与PKRP组比较,MOSES-HoLEP组的手术时间、膀胱冲洗时间、留置尿管时间、住院时间均缩短(P<0.01),具有切除标本质量多(P<0.05),血红蛋白下降幅度小(P<0.01)等优势,术后3个月随访PRV、IPSS、QOL和并发症发生情况也较PKEP组明显改善(P<0.05)。结论MOSES-HoLEP是一种安全有效的治疗BPH手术方式,可以明显减少术中出血量和手术时间,使得患者术后快速康复,并且降低术后并发症的发生。Objective To research the safety and effectivity of MOSES-HoLEP for treatment of BPH.Methods A total of 120 patients diagnosed with BPH were randomly divided into MOSES.HoLEP(n=60)and PKRP gruops(n=60)from October 2018 to February 2019.Operation time,the weight of resected prostate tissues,the decrease of hemoglobin,time of bladder irrigation,catheter indwelling time,length of stay,PRV,Qmax,IPSS,Q0L and the incidence of complications were compared.Results The time of operation,bladder imigation,catheter indwelling and length of stay were shorter in MOSES-HoLEP group than those in PKRP gruop(P<0.01).The weight of resected prostate tissues was more in MOSES-HoLEP group(P<0.05),however,the decrease of hemoglobin was more in PKRP gruop(P<0.01).Compared with PKRP gruop,PRV,IPSS,QOL and incidence of complications were significantly improved in MOSES-HoI EP group at three months post-operation(P<0.05).Conclusions MOSES-HoLEP is a safe and effective way to treat BPH,which can significantly reduce bleeding and operation time,make patients recover quickly and reduce complications.
