摘　　要：[目的]采用荟萃分析的方法,比较国人同期与分期双髋置换术的疗效。[方法]检索PubMed、Embase、Cochrane library、Web of science、中国知网、万方等数据库中关于双侧全髋置换同期与分期治疗的研究,检索年限为建库至2020年1月,依据纳入及排除标准筛选文献,提取数据,采用Review Manager 5.2软件分析。[结果]共纳入文献12篇,病例数共989例,其中同期组539例,分期组450例。荟萃分析显示,同期组在手术时间、住院时间、出血量、住院费用显著优于分期组(P<0.05),但同期组增加输血量(P<0.05)。两组在术后Harris评分、深静脉血栓、肺栓塞、术后感染率及术后神经系统症状发生率差异均无统计学意义(P>0.05)。[结论]同期双髋置换术较分期手术缩短手术时间和住院时间,降低出血量和住院费用,但增加输血量。[Objective]To compare clinical outcomes of simultaneous operation versus staged operation of bilateral total hip arthroplasty(THA)in Chinese by a meta-analysis.[Methods]The database,including PubMed,Embase,Cochrane library,Web of science,CNKI and Wanfang,were searched online to find the clinical control trials involving comparison of simultaneous bilateral THA with staged bilateral THA from database creating to January 2020.After screening and quality evaluation,the data were extracted for meta-analysis with Review Manager 5.2 software.[Results]Twelve articles met the criteria were enter this study with a total of 989 patients,including 539 patients of the simultaneous bilateral THA and 450 patients of the staged bilateral THA.As results of meta-analysis,the simultaneous bilateral THA proved significantly superior to the staged bilateral THA regarding to operation time,hospital stay,blood loss,and hospitalization expenses(P<0.05),whereas the former had significantly more blood transfusion than the latter(P<0.05).However,there were no significant differences between the two groups in postoperative Harris scores,occurrence of deep vein thrombosis,pulmonary embolism,wound infection and neuropsychiatric symptoms.[Conclusion]The simultaneous bilateral THA is superior to the staged bilateral THA in operation time,hospital stay,blood loss and hospitalization expenses,but inferior to the latter in blood transfusion.
