摘 要:为了解青岛地区鸡源大肠杆菌中碳青霉烯耐药基因bla NDM与黏菌素耐药基因mcr-1流行情况。2018-2019年从青岛地区共采集195份鸡直肠拭子和盲肠内容物,通过选择性培养基筛选目的菌株和16S rRNA菌属鉴定。采用琼脂稀释法测定菌株对10种常见重要抗菌药物的敏感性,通过PCR检测bla NDM与mcr耐药基因并测序。结果共计分离到186株大肠杆菌,其中鸡泄殖腔源98株,鸡盲肠源88株。药敏试验结果显示,分离菌株对阿莫西林、四环素、头孢氨苄耐药率均高于90%;多黏菌素81.2%、庆大霉素88.7%、氧氟沙星86.6%和头孢噻肟87.1%,呈现较高的耐药率;对替加环素(4.8%)则表现低水平耐药率。进一步分析发现,62.9%的测试菌株对8种以上药物耐药,表现多重耐药。耐药基因检测揭示了31.2%(n=58)大肠杆菌携带mcr-1基因,7.5%(n=14)携带bla NDM,包括1株bla NDM-1、1株bla NDM-9、12株bla NDM-5。其中6株大肠杆菌株同时携带了bla NDM和mcr-1,包括5株bla NDM-5+mcr-1和1株bla NDM-9+mcr-1。表明山东青岛地区鸡源大肠杆菌耐药菌检出率较高,多重耐药现象严重;且bla NDM和mcr-1存在共同传播。The purpose of this study was to understand the prevalence of carbapenem resistance gene bla NDM and colistin resistance gene mcr-1 in chicken-derived E.coli in Qingdao area of Shandong.From 2018 to 2019,a total of 220 chicken rectal swabs and cecum contents were collected from Qingdao,Shandong province,and the target strains and 16S rRNA bacteria were identified by selective medium selection.The agar dilution method was used to determine the susceptibility of the strains to 10 antibacterial drugs.The bla NDM and mcr resistance genes were detected by PCR and sequenced.A total of 186 strains of were isolated,of which 98 were from chicken cloacae and 88 were from cecum.The drug sensitivity results showed that the resistance rates to amoxicillin,tetracycline,and cephalexin were higher than 90%.The resistance rates of polymyxin,gentamicin,ofloxacin,and cefotaxime were 81.2%,88.7%,86.6%,and 87.1%,respectively,showing higher resistance rate.Low-level resistance was observed to tigecycline(4.8%).Further analysis found that 62.9% of the tested strains were resistant to more than 8 drugs,showing multidrug resistance.Drug resistance gene detection revealed that 31.2%(n=58)of E.coli carried the mcr-1 gene and 7.5%(n=14)carried bla NDM,including 1 bla NDM-1,1 bla NDM-9,and 12 bla NDM-5.It is worth mentioning that 6 E.coli strains carried both bla NDM and mcr-1,including 5 bla NDM-5+mcr-1 and 1 bla NDM-9+mcr-1.The study showed that chicken-derived Escherichia coli drug-resistant bacteria were detected at a higher rate in Qingdao,Shandong,and multidrug resistance was severe;bla NDM and mcr-1 coexisted.
